Roche has recently secured the CE mark, enabling it to enter the European CGM market and challenge Abbott and Dexcom. The CGM has AI that can predict glucose levels up to two hours in advance. There is a general consensus among Wall Street analysts that this new device is unlikely to pose a significant competitive threat to Dexcom and Abbott, and have expressed concerns about the need for initial calibration as a potential uptake barrier. However, Roche believes this unique feature sets them apart from its well-established incumbents as the glucose level prediction can help patients reduce stress and fear of hypoglycemia. A 33-year-old male patient presents for evaluation of abnormal FBS and PPBS levels that were observed as part of their routine annual health check. His FBS is 115 mg/dl, and his PPBS is 134 mg/dl. He has a family and personal history that is a high risk of developing lifestyle disorders such as diabetes. What is the next step in the management of this patient? Prescribe insulin therapy Prescribe metformin therapy Prescribe sulfonylurea therapy Prescribe lifestyle modifications Explanation: The first step in the management of T2DM is lifestyle modification, which includes dietary changes, regular exercise, and weight loss. These modifications can help improve glycemic control and reduce the risk of complications associated with diabetes. The Diabetes Prevention Program research paper found that lifestyle interventions, including diet and exercise, can significantly reduce the risk of developing type 2 diabetes in high-risk individuals. The study also showed that the benefits of these interventions can last for at least 10 years. Insulin therapy is usually reserved for patients with severe hyperglycemia or those who fail to achieve glycemic control with lifestyle modifications and oral medications.  Metformin is the first-line oral medication for T2DM and is usually prescribed along with lifestyle modifications.  Sulfonylureas are another class of oral medications that stimulate insulin secretion from the pancreas and are usually prescribed as a second-line therapy. However, they are associated with a higher risk of hypoglycemia and weight gain compared to metformin.
