Author commentary: Louis-Jean Masgnaux and Mathieu Pioche on "Endoscopic submucosal dissection assisted by adaptive traction: results of the first 54 procedures"   Endoscopic submucosal dissection (ESD) is a challenging procedure, but traction devices may improve access and aid completion. A disadvantage of existing traction devices is that they cannot be adjusted during the procedure. Editor's Choice: Lumen-apposing metal stents for the treatment of pancreatic and peripancreatic fluid collections and bleeding risk   A total of 264 patients with pancreatic and peripancreatic fluid collections (64% with walled-off necrosis) who underwent drainage with either a Hot-Axios or Spaxus stent were evaluated in this propensity score matched study. Evaluating jejunal disease with motility MRI & GIQuant. On Tuesday we looked a 'Crohn's in unusual places' and focused on the Jejunum - the middle segment of the small intestine: difficult to reach with endoscopy and easy to miss.  ⬇️ Below we can see motility MRI in the same patient. MRI is an example of an imaging technique that can see the whole bowel and therefore a good option for jejunal disease. However poor distention often impacts its sensitivity and specificity. 🤔 Can we do better? 👍 Yes! Top is before treatment and bottom is after treatment both evaluated with motility MRI and GIQuant. Here we can see a positive change in the score which suggests improvement in the disease without the need for IV contrast or spasmolytic. #ibd #crohnsdisease #gastroenterology

🩸💡 Wondering if you should embolize a non-hypertrophic bronchial artery in patients with hemoptysis? Here's a case that sheds some light! 👩⚕️ Meet a female in her '50s with no medical history, presenting with hemoptysis. CT scan revealed intrabronchial content and signs of hemorrhage, especially in the left lower lobe. 🔍 Despite no pathological bronchial artery seen on CT, in collaboration with our pulmonologist colleagues, we decided on empirical embolization of the left bronchial artery. 🩺 The small, fragile artery was catheterized and embolized with particles. But then, whoosh! Amongst others because of the increased peripheral resistance the thrombus was blown away , revealing an obvious fistula to the left main bronchus, as seen in the clip. 💡 We further embolized with glue (lipiodol:histoacryl) with a technically good result. In some cases, empirical embolization of non-hypertrophic bronchial arteries can provide relief for hemoptysis. Always consult with your team consisting of interventional radiologists and pulmonologists for the best approach! No evidence of haemorrhagic cyst.  Non-obstructive bilateral renal calculi are seen. Similar cystic lesions are also seen in both lobe of liver. Rest of the liver parenchyma is unremarkable. 【Lumbar Synovial Cyst, Literature Review: Challenges of the Surgical Management, the Role of Minimal Invasive Techniques and Endoscopy】 Full article: https://lnkd.in/g9ik9tNA (Authored by Marcel Sincari, et al., from Centro Hospitalar Tondela-Viseu (Portugal), etc.) The synovial cysts are contiguous with facet joints, arising through a defect in the wall of the joint capsule, caused by instability, degenerative changes, trauma and inflammatory diseases like rheumatoid arthritis, causing narrowing of the spinal canal and provoking neurogenic claudication or radicular symptoms. The management of symptomatic #lumbar_synovial_cysts is still a challenge and a matter of debate. This paper provides an updated review of a literature about treatment of juxta facet synovial cysts and presents some cases of synovial cysts treated surgically, focusing on endoscopic solution. #Lumbar_Decompression #Facet_Joint

Editor's Choice: High quality colonoscopy: using textbook process as a composite quality measure   A Delphi consensus process was completed by 20 international experts to propose a definition of “textbook process” (TP) as a new composite measure for optimal colonoscopy procedure. Ten items were finally identified and applied to clinical practice using data prospectively collected in two endoscopy services in the Netherlands. TP was achieved in 5962/8227 colonoscopies (72.5%) and rates varied significantly (41.0%–89.1%) among the 48 endoscopists.
