Home hospice care is the same for all dying patients, but unlike Mr. Carter’s family who can afford to augment the hospice team with full-time professionals, regular folks must find enough volunteers to fill in a 24-hour day for months. It’s important for hospice consumers who are considering hospice-in-the-home to be aware that the interdisciplinary team (which is required to make a visit once a week) consists of an RN, SW, Certified Nursing Assistant, Chaplain, and a volunteer. They often spend more time with a patient in the beginning (as care plans and documentation are initiated) and in the final hours. But most family members undertake the majority of the work for their loved ones. Each team member is only required to make one visit per week. Incidentally, hospice gets paid $200 per patient, per day, whether they visit the home or not. However, many families do not feel comfortable undertaking the brunt of intimate care or medication dispensation or have the time to engage in that much caregiving. End-of-life care is tough on families, especially when a loved one is close to dying. Instead, families often resent spending their last months stressed and exhausted with caretaking. Bigger families, or those with volunteers from church, or neighbors may be able to help pitch in to cover the round-the-clock staffing that an inpatient hospice residence or nursing home would provide. Unfortunately, hospice residences have around 20 beds, so patients can only be admitted within a few weeks of dying. For my parents, that was the ideal situation and they had a good dying experience. Wealthy families like that of Mr. Carter can afford to hire the complementary staffing that is needed in addition to the hospice team. In the right circumstance, dying at home can be a beautiful thing, but it’s not for everyone.
