NYC Health + Hospitals today announced a multi- million-dollar upgrade to new, state-of-the-art medication management technology in its hospitals, which dispense over 22 million medication doses a year to patients. The centralized cloud-based Omnicell equipment interfaces with the medical record and purchasing systems, while automating critical pharmacy processes associated with storing, tracking, packaging, and dispensing medication. The system uses barcode scanning and patient profiles, among other safety features, to decrease medication errors, which – according to the Agency for Healthcare Research and Quality – is one of the most common causes of patient harm and can occur at any time during the medication administration process. Alhamdulillah I’m pleased to announce my first authored article, titled “Analyzing Override Patterns in Profiled Automated Dispensing Cabinets at a Tertiary Care Hospital in Saudi Arabia.” which recently published in the prestigious Journal of the American Pharmacists Association by Elsevier . This article provides valuable insights into the fields of pharmacy automation and medication safety. I was honored to receive the following positive comments from the esteemed journal reviewers: ✳ “This study contributes evidence to an area of pharmacy where there is little recommended in terms of benchmarking and metrics.” ✳ “This study adds to the literature, taking the subject of ADC overrides from a different angle, looking at the details of the situation vs just a yes or no approach.” Key insights ✅ The target ADC (Automated Dispensing Cabinet) override threshold of 5% is inadequate to demonstrate sufficient adherence to the standard elements of appropriate ADC overrides or effectively prevent medication errors. ✅ While the overall 5% target threshold was achieved, multiple inappropriate override practices were uncovered, such as missing physician orders, undocumented verbal orders, non-urgent physician orders, missing or delayed administration records, and unrecognized override errors. ✅ Stricter override limits for profiled ADCs, ideally well below 5% can enhance medication safety, particularly with 24-hour pharmacy services and fully integrated CPOE system with ADC system. ✅ The average pharmacist verification time in CPOE system integrated with ADC system is only 5.6 minutes. This may allow nurses to address urgent medication overrides promptly. ✅ Multidisciplinary surveillance of ADC overrides is considered essential. A smaller proportion of health-system pharmacists can independently prescribe a medication pursuant to a diagnosis than they did from 2020 to 2023, according to provisional survey data presented at the ASHP Midyear2024 Clinical Meeting & Exhibition, in New Orleans. The percentage of hospital #pharmacists with such prescribing authority ranged from 21% to 31% in previous years, and stands at 17.5% in 2024. I bet it’s higher for non-health systems pharmacists.

Pharmacy technicians reported a substantial reduction in the time needed to check and organize #medications, from one hour to five minutes per tray. Adopting RFID pre-labelled medications #reduces #errors and frees pharmacists from other critical tasks.
