I'm excited to see the growth of multisector partnerships aimed at addressing health-related social needs (HRSNs). These collaborations between healthcare providers, community organizations, and other stakeholders are driving real change. By focusing on issues like food insecurity, housing instability, and mental health, states are showing how multisector efforts can improve health outcomes, reduce costs, and address health disparities. Through innovative models like Accountable Communities for Health, we're seeing a shift towards long-term community health improvement and equity. These efforts demonstrate value over short, medium, and long terms: 💠𝐒𝐡𝐨𝐫𝐭-𝐭𝐞𝐫𝐦 (𝐇𝐑𝐒𝐍 𝐃𝐞𝐥𝐢𝐯𝐞𝐫𝐲 𝐒𝐲𝐬𝐭𝐞𝐦): Immediate ROI through improved health outcomes, reduced costs, and utilization. 𝐄𝐱𝐚𝐦𝐩𝐥𝐞: Greater Cleveland United Way: Medically tailored meals for older adults with chronic conditions, focusing on ROI for investors. ‘Currently, use of digital technology is mimicking silos in the system, and people are frustrated at having to piece their care together. This fragmentation is partly due to the funding and approaches to digitalisation replicating organisational silos in the system and the technology then copying this.’ Another frank and helpful review from the Kings Fund on the potential for digital to enable community health and social care. One of the main priorities for the strategy and performance teams in Midlothian is to progress the DSAs we need and release the IG bottleneck across much of integrated health and social care to ensure our communities have access to the quality support they deserve. https://lnkd.in/e7PbB7wG
