🎉 Great news!! My capstone, as part of my doctoral project through Shenandoah University Doctor of Medical Science program, has been a ton of hard work, but it has paid off in the form of a $10k grant from the AAPA PA foundation awarded to the non-profit faith-based clinic where I did my first clinical rotation in PA school. Really excited about working with TOMAGWA HealthCare Ministries to bring trauma-informed women’s healthcare to the community. 💜 There she sits on the examination table, a thin paper gown offering little solace against the starkness of the room, making her feel almost as if she is wearing nothing at all. This subtle exposure intensifies her feelings of vulnerability. The addition of cold, metal stirrups only accentuate her discomfort. Placing her feet in them positions her in a way that is not just physically uncomfortable but also psychologically distressing. The pose commands an unbalanced power dynamic, emphasizing her lack of control at that moment. For any woman, this can be an unsettling experience. But for those with histories of trauma or adverse childhood experiences (ACEs), the feelings can be magnified exponentially. Being in such an exposed and powerless position can serve as a potent trigger, potentially evoking distressing memories and sensations. It can remind her of when her autonomy was taken away, bringing feelings of helplessness, fear, or even re-traumatization. This process, meant for her health, can inadvertently challenge her emotional well-being, highlighting the need for a more sensitive, trauma-informed approach. Now imagine a community where every woman, regardless of her past, feels seen, heard, and understood by her healthcare providers. She walks into a clinic not as another statistic or patient number but as an individual with unique experiences and needs. This is the vision TOMAGWA is striving towards. Too many women in four specific Texas counties carry the weight of past traumas, often amplified by socioeconomic struggles. Their experiences are more than just footnotes in their histories; they influence health outcomes, shape interactions, and can cast long shadows over their futures. With this grant, TOMAGWA and the PA Foundation will champion an approach to women's healthcare that does not just treat symptoms; it delves into the root, understanding, and addressing the impact of trauma on women's health. Both organizations will seamlessly blend initiatives from tailored clinical protocols and ACE screenings to group prenatal care sessions, all underpinned by the latest research and best practices. I would like to thank everyone who collaborated to make this happen and to shape a future where every woman feels cherished, supported, and, most importantly, truly seen.

The National Center for Children's Vision and Eye Health at Prevent Blindness has opened nominations for the 2024 Bonnie Strickland Champion for Children's Vision Award. The Award recognizes significant efforts by an individual or group of individuals to improve public health approaches for children’s vision and eye health at the state or national level that are replicable and create sustainable change. It was created to commemorate Bonnie Strickland and her groundbreaking work to establish a comprehensive system for children’s vision in the United States. Nominees for the Award may include individuals or a group comprised of diverse stakeholders, including family/caregiver and community leaders, who are implementing innovative changes to improve children’s vision in the United States. This looks like I have written it :) I would add: Considering prevention as a distinct focus is important, as it often gets overlooked when combined with care. Supporting those living with dementia is crucial, but it is equally important to focus on those not affected. I would like to share with you about the non-profit organization that I started called Sunrise NPPA Derm Inc. It was founded with the principles that physician assistants (PAs) and nurse practitioners (NPs), and physicians alike, should work together to maintain the ethics, values, and standard of care that we were taught when we went through our training for our respective professions. Sunrise NPPA Derm believes that patients deserve the quality care that we all want to provide. Nobody--not us, not our friends, not our parents, not our children, not our neighbors, nobody-- would want to be treated like just a number when it comes to our health. Today, the healthcare landscape is changing quickly. Private equity is buying out healthcare practices, and large corporations are taking over our local specialists and primary care providers. Representation in medicine literally saves lives: "Only 2.8% of all physicians in the U.S. are Black Women. This 4-part limited series aims to change that. But why does it matter? Black infant mortality is more than double the rate for white infants. This disparity drops by more than 50% if the pediatrician is also Black. All pediatricians want their patients to thrive, but the impact of representation is undeniable. The lack of diversity perpetuates health inequity in other areas too. The house of medicine needs to reflect the nation we serve. We are using the power of storytelling to move the needle towards true representation in medicine." "Faces of Medicine is a project centered on the paths of Black female physicians in the United States. The documentary series starts with recognition of the trailblazing work of Dr. Rebecca Lee Crumpler, who in 1864 became the first Black female to earn an MD in the United States. We explore the journeys of Black women practicing medicine today as captured in live interviews. The book will expose readers to even more details about each doctor. We are sharing the stories of how we came to choose careers in medicine, made it through challenges along the way and celebrated successes. Some of these doctors are first generation college graduates and others are second generation physicians. Some immigrated to the U.S. as children while others can trace 200 years of ancestry in the deep south. Their journeys will be woven into a quilt representing unheard voices and untold stories. The hope is that any Black girl or woman can find themselves in our stories and know what is possible. All viewers will gain a better appreciation of what doctors of color experience. Those who are interested in improving health equity will see perspectives they may not otherwise be able to access. Anyone who interacts with healthcare providers — which is to say, all of us — will have an opportunity to shift their bias." Personal interactions matter in private practice and small businesses alike. Daniel Choi has a wonderful post on key points to successfully run an independent practice in todays environment. It was very forward thinking and innovation based. I agree with all of it. However, I may still be old fashioned in that personal engagement still matters. It may even matter more now than ever. It is a differentiator that the big players can’t do well. 1. Engage your community. Churches charities and softball teams We make a point of working with small businesses. It surprising how some of the pressures they experience are the same as yours. That empathy and understanding is important for gaining trust in your community 2. There are a number of physicians in your community and providers that work outside the larger systems. Take the time to engage and work with them. We have a growing number of concierge physicians that need and want personalized interactions for there patients 3. Let your patients help. My number one referral source is still simple word of mouth. Encourage and foster that. Most of my patients are proud and honored to help. Most of my early patients, take pride in helping build the practice. 4. Be nice to those around you. Simple but meaningful engagements are great ways to grow your patient base. 5. Diversify. Offer services that are unique to you. We offer nutrition, functional fitness, bone health, etc. And do what Daniel says…. Also having mentors like Alok Sharan Venu Vemuri, DO is helpful. I truly believe that there is an opportunity to stuff that is really unique in the independent space…. I am not a medical person or a psychology trained person, I write based on my knowledge and understanding. for many #carers and those who experience loss, goal setting in recovery, can be confusing and very daunting.
