Yes, the hospitalist system has resulted in the further siloing of healthcare. So much so that hospitalists (as well as ER docs, intensivists, and other specialists), in their almost exclusive focus on acute stabilization and life-saving curative care, often miss the forest (disease progression) for the trees (acute illness), and sometimes, the elephant in the room (death). The textbook I’m writing seeks to right this wrong, which stems from the medical training pradigm, which does not teach about terminal decline, how to recognize where a patient is in their end-of-life chapter, how to evaluate using hospice criteria (which need to be retooled), or about hospice care as a valid, and often preferred, treatment modality. We have a long way yet to go on patient-centered care. The first step is having a medical community trained to recognize and discuss disease progression and end-of-life care.

The hospitalist system, which is here to stay, has increased the divide between inpatient and outpatient care. One way to address this is through the use of care consultations by primary care providers through telemedicine while their patients are in the hospital. This would require coordination between the hospital and the PCP office, communication between the hospitalist and the PCP, and a change at CMS to allow the PCP to bill for that time. The benefits would be better clinical outcomes, better post-discharge care coordination, improved patient satisfaction, and lower costs. Interesting read and take. Higher patient satisfaction, in many cases higher cost of care. It comes to balancing care, cost, providing patient education and choices. Healthcare is ever evolving and clear communication is key to ensuring balanced delivery and outcomes.

The Health Minister Robin Swann made a statement to the Assembly today on waiting lists. He said: "At the end of September 2023 – the most recent validated public figures - there were 430,000 patients waiting for a first consultant-led outpatient appointment; 115,000 patients waiting for inpatient or day case treatment; and just under 190,000 patients were waiting for a diagnostic test." The Minister warned that “piecemeal interventions” alone will not be sufficient. “Tackling our lengthy waiting times is undoubtedly a long-term issue that requires sustainable and recurrent funding, workforce development and system wide transformation. The health and social care system is complex, and waiting lists are just one component. As such, they cannot be addressed in isolation. “Other fundamentals of the system must be considered and addressed simultaneously, including issues such as workforce pay, pressures and capacity in secondary, primary and community care, and ensuring we maximise the capacity we have within the system." The Minister added that initiatives for which confirmation of sustainable funding would allow further development include: Enhancement of sustainable workforce capacity through expanding skills mix and development of specialist nurse roles and Allied Health Professionals across a range of specialties; Expansion of pre-op assessment services to improve theatre productivity; Building our diagnostic capacity to reduce the reliance on IS capacity; Building capacity and throughput across regional elective care services such as urology, paediatric and orthopaedics.

There’s no denying our health-care system is in crisis, and it’s not working for anyone – patients or healthcare workers. We need team-based solutions to make medicine more effective and efficient.
