Occupational therapy can improve our ability to carry out everyday tasks, maintain independence, enhance overall life quality, and help protect against painful joints or severe arthritis symptoms. RA is a progressive disease that comes with a variety of comorbidities. As my body and my health changes, OT has helped me along the way. Although I was acquainted with physiotherapy, I initially lacked an understanding of the role occupational therapy played after my diagnosis and when considering long-term disability. I had no awareness that occupational therapists (OTs) could help address the myriad challenges stemming from my arthritis diagnosis and its associated comorbidities. OTs can assist in various aspects of life, such as parenting, socializing, household chores, intimate relationships, and achieving work-life balance. Reflecting on a recent discussion with some of our OTs, I’ve been thinking about the role of client-centred care in Occupational Therapy. It’s a topic close to my heart and a core principle in our field. I’d like to discuss how it can transform our approach to care. Navigating client-centred care is both an essential practice and a continuous learning journey. While we all understand the concept, applying it effectively requires ongoing reflection and adaptation. At its heart, client-centred care is about truly understanding our clients' unique experiences, values, and goals. It's more than just using our clinical expertise; it's about listening deeply and valuing their input. This collaborative approach transforms therapy into a personal and meaningful journey for each client. One of the key challenges is balancing our knowledge with client autonomy. While our training provides us with a strong foundation, it's crucial to set aside assumptions and engage in genuine dialogue. This means not just asking questions but creating an ongoing conversation throughout the therapeutic process. Co-creating goals with clients ensures the therapy aligns with their life context and priorities. Flexibility is another critical component. Each client's story is unique, and our methods should reflect that diversity. Being open to adjusting treatment plans as we gain new insights can significantly enhance the effectiveness of our interventions. This adaptability not only respects the client's journey but also makes our role more dynamic and responsive. Building trust is foundational in this process. It requires us to refine our communication skills, deepen our cultural competence, and consistently demonstrate empathy. These elements are vital for making clients feel heard, valued, and supported. In the end, client-centred care is about empowering clients to take an active role in their own rehabilitation. It’s about recognising that they are the experts in their own lives, and our job is to support them on their path to achieving their goals through understanding and care. It's a Friday Jargon Buster! You say Tomato...or in this case Initial Needs Assessment (INA) and we say Rehabilitation Needs Assessment (RNA). Why? I hear you ask! TT1st use the term RNA because we find it more rehab focussed and whilst it looks at and addresses a client initial needs. Our RNAs look far beyond the now. Your clients RNA can be completed within 2 weeks of instruction depending on the Clients availability! The Specialist Rehabilitation Occupational Therapist (SROT) will be with your client for between 2-3hrs during which time they evaluate the clients’ current capabilities. This is achieved using a combination of structured questions, clinical assessments, observations and where appropriate consultation with others involved in the client’s rehab or care. Throughout the RNA our SROTs monitor your client for any signs of distress or anxiety to ensure their comfort and well-being. Within 10 working days post assessment, a detailed easy to read report outlining recommendations and associated costs is provided to the referring party. Additionally, the SROT who conducted the assessment can be available for a video call with the referring parties to discuss recommendations and address any questions you may have. PSP and CBD Treatments The most effective treatments are the therapies: physical therapy, occupational therapy, and speech therapy. Therapies are pinpointed on prevention of falls and aspiration pneumonia. Exercise is key for fall prevention and maintaining mobility. The medication that can be prescribed and any other treatments are focused on a few symptoms that may respond to medication or treatments such as depression, urinary incontinence, and dry eyes. Those with PSP-parkinsonism do have some response to levodopa medication. There is no evidence that any of the Alzheimer’s medications are helpful with the dementia in Richardson’s syndrome. There is no evidence that any of the Parkinson’s medications (levodopa therapy) or Alzheimer’s medications are helpful in CBD/CBS.
