Our healthcare system in the United States is broken in a lot of ways. Getting a cancer diagnosis shouldn't mean that you go into debt for the rest of your life. When you're trying to fight cancer, the last thing you should be worried about is whether or not you can afford it. This is why health equity is so important to me. When we tackle inequities, we see better patient outcomes. “Cancer survival rates over recent years have shown improvement across races and ethnicities, so by acknowledging and addressing barriers to care, further improvements toward equitable access, treatment, and outcomes can be realized for all,” says Lori Pierce. If we want to see even more positive change when it comes to cancer care, we have to see an embrace of health equity throughout all medical care. This is a needed trend which I know is coming. Obviously this will have nuanced hurdles as hospitals will be hesitant to be responsible for patients outside of their walls and even moreso to give up said incentives to keep patients inside their walls (virtual or otherwise.). We will be eager to hear more about the planned strategies and technologies available to decide which patients and which treatment te are most suitable for this paradigm shift. Injectables and at home supportive infusions are obviously a pathway. Supportive care teams to help assess and guide need for urgent care or assessment will be crucial. Mobile services and clinical availability will likely round out the delivery and peri-therapeutic events and issues. And of course clinician buy in and guidance will be an evolution. Ultimately the time is ripe and right for this transition. The politics and delegation of responsibility will need creative and autonomous clinicians and systems willing to improve quality, increase efficiency, and reduce costs. The incentive model will need to re-evaluate where we are and seek to remove historic barriers to allow the logistics to flourish as a priority.

Over the past decade, I've focused deeply into the implications of value-based care and quality measurement for the pharmaceutical industry, particularly in terms of its strategies for market access, evidence generation, government policy, health equity, and patient advocacy. Simultaneously, I've maintained a strong interest in addressing fundamental flaws within our healthcare system, aiming to instigate positive systemic change. With a scientific background and extensive involvement in clinical trials throughout the early years of my career, I've had the opportunity to focus on exploring more recently how our healthcare data infrastructure can evolve to align with the Quintuple Aim objectives. Specifically, I'm passionate about effecting change to improve patient outcomes, enhance patient and provider experiences, reduce the total cost of care, and advance health equity. I'm excited to currently be involved in work that contributes to the establishment of a foundation for data interoperability in cancer care. Collaborators across health care, public health, industry, and technology sectors are working together on initiatives like the United States Core Data for Interoperability Plus Cancer (USCDI+ Cancer) to support whole-person, precision care incentivized by value-based care and quality measurement, as is talked about in the article below. This work aims to build infrastructure to facilitate meaningful data sharing across clinical care, public health, clinical research, and other systems involved in patient care and services. In my current role as a Senior Consultant (ONC Contractor) through Next Level Health Innovations, LLC, I'm actively involved in implementing and advancing USCDI+ Cancer. Our near-term goal is to ensure that real-world use cases, such as the Enhanced Oncology Model, cancer registries, immune-mediated adverse events, and clinical trial matching, benefit from enhanced data interoperability. Through this work, we aim to foster a more cohesive and effective healthcare ecosystem.
