“The most basic of all human needs is the need to understand and be understood. The best way to understand people is to listen to them.” ― Ralph G. Nichols “Speech communities,” as described by author and linguist Paul Roberts, are characterized by their ability to negotiate meaning through shared language, which creates a layer of identity and belonging. Individuals with rare diseases and their families indeed form a speech community, and one of our common terms is “diagnostic odyssey,” a journey to diagnosis which is usually lengthy, complex, and traumatic.  It’s inevitable that a rare disease diagnosis will entail pain and grief. But I’m convinced that the trauma involved in the diagnostic odyssey can be reduced and mitigated with access to more evolved diagnostic tools and cross-disciplinary collaboration that is rooted not in ego, but in ethics and empathy. This narrative is the personal story of our family’s diagnostic odyssey and Evren’s diagnosis with Acid Sphingomyelinase Deficiency (ASMD), a rare autosomal recessive genetic disease. Even after many years, writing it evoked sadness. Nevertheless, I wrote it for its potential value for teaching lessons about communication and gaining insights into trust. It was not written for members of the rare community who have their own trauma to cope with, but for those outside of the rare community so they might experience a diagnostic journey from a new perspective. Personal narratives can serve as agents of change and of hope. In this context, that is hope for less trauma for individuals and their family members, and also hope that the term “odyssey’ will become lost to common usage after replacement with a term that implies something far less fraught and lengthy, maybe something like “path.”  Thank you to the leadership and staff of both Project Hope and Health Affairs for honoring the powerful role that personal narratives have in sustaining interest in healthcare policy reform and fostering improved practice. It is the narrative that best humanizes terms like “diagnostic odyssey” and illuminates stakeholder perspectives that would have otherwise been invisible, unacknowledged, or forgotten. The sharing of our story is my contribution, alongside those of professionals in the social and natural sciences, to the new diagnostic pathway I envision for future rare families. In his poem “Song of the Universal,” Walt Whitman reflects my vision in these lines: “Embracing, carrying, welcoming all, thou too, by pathways broad and new, To the ideal tendest.” 

In 2021, my mom passed away after contracting COVID-19. Reflecting on her health, I believe a lack of regular exercise played a significant role in her overall condition and her ability to survive the virus. This is why the American Journal of Preventive Medicine 2023 Article of the Year "Associations of Physical Inactivity and COVID-19 Outcomes Among Subgroups" deeply resonates with me. The study, involving over 200,000 patients with positive COVID-19 diagnoses over two years, found that physical activity prior to infection was linked to less severe outcomes across various demographics, genders, and clinical presentations. This research emphasizes another crucial reason to exercise. The health benefits of exercise, including improvements in mental health, disease reduction, and overall lifestyle, have been well-documented. Here’s yet another compelling reason to stay active. Exercise truly is free medicine. Dr. Robert Sallis, MD, raises an important question: How can we take advantage of this free medicine and incorporate physical activity into our daily routines, taking into consideration our busy schedules, environments and the availability of safe spaces? As a Project Manager, I've faced my own challenges balancing work, family, and personal health. Exercise requires intentionality and consistency. Over the past two months, I’ve made it a priority to go on walks with my son. We started small, stepping out into the driveway, and gradually increased our walk times. This routine not only helps me stay healthy and mentally sound but also allows me to set a positive example for my son and work towards a healthy lifestyle.
