Wrapping up a very inspiring day at the American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR) Day on the Hill event 2024 edition. More than 75 clinicians from over 25 states converged upon Washington, DC to engage in some very grassroots advocacy. Our asks are simple: remove the barriers to increasing participation rates in CR/PR by eliminating the reimbursement disparity between services provided in a physician office versus a hospital outpatient department that has blocked program expansion since 2015. Second, make permanent the waivers granted by CMS during the PHE that allowed for virtual delivery of CR/PR services. Without alternative delivery models we can’t hope to reach the millions of eligible patients that reside in rural areas far from brick and mortar programs. Please consider sending a letter to your Representative and Senators asking for their support. Even if you’ve done so already (and thank you if you did) letting them know this is still important won’t hurt. How many times did you have to ask Mom or Dad for something you wanted? In a recent study, Dr. David Levine Clinical Director for Research and Development for Mass General Brigham Healthcare at Home found that among thousands of episodes of at-home hospital care, outcomes did not differ based on a person's race, ethnicity, or presence of disability. According to Dr. Levine, "It was reassuring to see that there were not clinically meaningful differences in outcomes across marginalized populations, because we know that there are massive disparities in outcomes for traditional hospitalization. This suggests home hospital can really reach a diverse group of patients and families." This study provides valuable insight into the potential for at-home hospital care to bridge the gap in healthcare disparities. By providing care that is equally effective for all patients, regardless of their race, ethnicity, or disability status, the growth of at-home hospital care can help create a more equitable healthcare system.

"People over 65 use more health care than other age groups and make up nearly half of the hospital admissions. But there are just 7,300 board-certified geriatricians in the United States, which is fewer than 1 percent of all physicians," writes Stern. Important Update for Physical Therapy Practices Starting September 1, UnitedHealthcare (UHC) is expanding prior authorization requirements for follow-up PT visits in office and outpatient hospital settings. This change affects certain plans, but there is some relief for practices that qualify for UHC’s new Gold Card program beginning October 1. Gold Card providers won’t need prior authorization for specific services if they meet evidence-based guidelines. Additionally, UHC’s Optum Healthcare Solutions will require a patient summary form for prior authorization of treatment visits following an initial evaluation for certain plans. This change also takes effect on September 1. The APTA is actively advocating against these new policies and seeking clarifications. Stay informed and voice your concerns to UHC and Optum.
