The risk of symptoms is probably greatest in children treated for spasticity, but symptoms can also occur in adults treated for spasticity and other conditions, particularly in those patients who have an underlying condition that would predispose them to these symptoms. In unapproved uses and approved indications, cases of spread of effect have been reported at doses comparable to those used to treat cervical dystonia and spasticity and at lower doses. This systematic review assesses the effect of routine screening for infectious dental foci on the incidence of postoperative PVE. The Cochrane Library, PubMed and EMBASE databases were all searched for relevant primary studies. The GRADE system was used to rate the studies. Three primary studies with a very low level of evidence were included in the review. No significant difference was found between the incidence of PVE in groups with or without routine screening for infectious dental foci prior to heart valve replacement. The results of this review do not support or disprove the need for preoperative screening for infectious dental foci to prevent PVE in patients undergoing heart valve replacement. Treatment of osteonecrosis of the jaw with bluem oral gel. 🇧🇷 By Tatiana Deliberador. 👉𝐀𝐛𝐬𝐭𝐫𝐚𝐜𝐭 Medication-related osteonecrosis of the jaw (MRONJ) can be a debilitating complication that can arise in patients who took or are taking antiresorptive (including bisphosphonates) or antiangiogenic agents, leading to visible bone or a fistula that continues for more than eight weeks, without any history of radiotherapy. This clinical case aimed to describe the treatment of MRONJ with topical active oxygen therapy using bluem oral gel. A 63-year-old female patient that had been taking weekly sodium alendronate (70 mg) for four years by oral via, presented discomfort and #implant movement in the 46 region, by that underwent #surgical extraction of the implant. After three months the patient returned and was diagnosed with MRONJ. Initially, conventional therapies were performed, including surgical debridement and antibiotic therapy, but without #success. The patient still had clinical signs of osteonecrosis six months after the implant extraction. The entire #socket was then filled with bluem oral gel by topical application. The patient was instructed to continue applying the gel to the region every 8 hours for 15 days. After this period, the patient returned, and it was observed that the #wound was in the healing process, with the presence of epithelialized tissue and without bone exposure. The 2-year clinical follow-up showed the lesion had healed entirely, and a new implant was installed. After the osseointegration period, the final prosthesis was placed. The patient remains under clinical follow-up. How often do we take the time to ensure our patients truly understand their medications? 💊 Recently, I had an experience with a patient being discharged after a total hip replacement. The patient's family member was visibly stressed about the medication list and was particularly concerned about celecoxib, mistakenly believing it was another narcotic. They were adamant about avoiding around-the-clock narcotic use, misunderstanding its purpose. Taking the time to explain that celecoxib is not a narcotic, but rather an anti-inflammatory medication that not only helps with pain but also prevents heterotopic ossification after surgery, made a significant difference. Once they understood its importance, using simple terminology, they felt calmer and more open to the discharge medication regimen. This experience reinforced for me the crucial role of patient education. By providing clear and thorough information, we empower our patients and their families to make informed decisions about their health. Biofilms produce a matrix of polymeric substances which enhance their resistance to antimicrobials. But understanding how biofilms form and how they can be managed is key to the development of safe and effective medical devices.
