Daily Critical Care Pearls #172: Heparin-induced Thrombocytopenia - Part 2 📌 For a patient diagnosed with HIT who has just begun warfarin, you can give full dose anticoagulation with an appropriate agent and hold the warfarin and reverse it with vitamin K. 📌 For a patient with obesity, it is preferred to use argatroban as the chosen anticoagulant for managing HIT. Monitoring aPTT 2 or 3 hours after the rate change is appropriate based on the half-life, but consider extending this in patients with hepatic dysfunction to account for the longer half-life in such patients. #criticalcare #clinicalpharmacy #medicine #pearls

In this Diabetes Care publication, we take a deep-dive into CGM data from two once-weekly icodec trials to answer pertinent clinical questions. Specifically, the hypoglycemia results are highly reassuring, given the context that the ONWARDS 2 and 4 trials included people with longstanding type 2 diabetes switching from basal alone or basal-bolus regimen ie those who would be deemed to be at high risk for hypoglycemia. Catch this condition early to improve patient outcomes and even save patients' lives! A case report published in the JDPA ("Calciphylaxis: complexity from presentation to treatment") found that patients with calciphylaxis, also known as calcific uremic arteriolopathy, typically present with areas of purpura and ulcerations where deposits of calcium have formed within the blood vessel walls of subcutaneous tissue. It can be challenging to diagnosis calciphylaxis as there are no conclusive serologic, radiographic, or visual signs that the disease is manifesting. The authors report two cases of calciphylaxis, one associated with renal failure and the other a history of warfarin use. Common risk factors for calciphylaxis include female patients, over 60 years old, BMI of over 30, diabetes, use of vitamin K antagonists (like warfarin), and elevated serum phosphate levels. Diabetes does not represent a contraindication to compression therapy in patients with venous leg ulcers even with additional arterial occlusive disease (excluding critical limb ischemia), and it does not play a negative prognostic role regarding ulcer healing. It was shown that compression therapy may be safely applied in diabetic patients with recalcitrant ulcers, even in the presence of moderate peripheral arterial occlusive disease. Additionally, such therapy did not result in un­wanted effects, but just a light, not significant healing delay compared to non-diabetic patients.
