Health care providers can only treat patients if we can reach them. At Advocate Health, we find ways to remove the barriers that patients sometimes run into when seeking care. That can mean expanding eligibility for financial assistance, connecting patients with resources that address unmet social needs, and even deploying mobile clinics that literally meet patients where they are. As our health system grows, we’re bringing more resources and expertise back home to the communities where our patients live. Find out more about how we’re bridging gaps in access to care and investing in programs that lead to more equitable and resilient communities: https://bit.ly/3LjftZg

Interesting article looking at the challenges that face Primary Care and how we stack up against other nations. There is no one size fits all when it comes to medicine and no one country has it all figured out. From my perspective one of the biggest challenges I see is how uncoordinated our care is here in the US. Right hand very rarely knows what the left hand is doing and it is detrimental to our healthcare. There is a lot of talk of care coordination, but very little in practice. If you want good healthcare, you have to be your best advocate, or find someone who is willing, able and capable because the system is not set up to successfully coordinate care. #patientadvocate #healthcareadvocate #agingparents #CareManager #GeriatricCareManager #nursenavigator #havenhealthcareadvocates #youarenotalone

As folks "try" to move to value based care the corporate robbers are moving faster to extract profits from what is supposed to be a care system, but has become a system of transactions, in which instead of transactions supporting success for care, care is now the excuse for billing the transactions. This is progressing along, and has for 45 years. Stopping it requires reinventing healthcare altogether: healthcare access should be a RIGHT, with everyone covered for the vast majority of care, and every community with adequate primary care, regardless of location. National consensus will require a grassroots movement of care providers and patients. It's past time.

There’s simply no evidence that 60 years spent building vertically and horizontally integrated, segregating, rationing and inequitably subsidized mega-profitable health insurance conglomerates has improved most Americans medical care access, outcomes or costs. #tripleaim #valuebasedcare #vbc #healthcare #medicareadvantage #healthinsurance #sdoh

Clearly Bismarck Healthcare used in Germany, Switzerland, Japan and other democratic nations is the answer enabling access for all citizens to quality affordable private insured and private #ffs manufactured medical care with more private insurers and private hospitals per capita than in America and better clinical outcomes and costs than in the USA. How’s this achieved, simply by making health insurers profit-neutralized utilities instead of luxuries. Read about Bismarck Healthcare Here; https://lnkd.in/ecUwsHWP

When we enhance "patient engagement,'' we create positive health outcomes for those in our care. Read why this is a vital element of the Integrated Primary Care (IPC) Model - https://lnkd.in/eZtvmBUG. #Healthequity #valuebasedcare #valuebasedhealthcare #healthcareinnovation #healthcareequity #patientcare #healthandwellness #healthcenters #communityhealthcenters #communityhealth #integratedcare #PrimaryCare
Did you know? That the Integrated Primary Care (IPC) Model consists of seven domains which serve as the lens through which health equity can be measured for all patients? And did you also know that the IPC Model has emerged as the blueprint to guide Federally Qualified Health Centers (FQHCs) as they conduct organizational reinvestments to address their pursuit of Value-Based Care (VBC)? "It’s no surprise to those who work in public health that retailers have not been able to succeed in the primary care landscape. Although their intentions may be genuine, retailers are in the business of making money, but that’s not exactly how it often works in healthcare, especially when it comes to providing primary care to Florida’s most vulnerable populations." In our recent blog, we highlight the importance of local community health centers and the impact they have. Read more about it below!

We cannot address #healthcare disparities without accurate data about them, as Kara Hartnett recently reported in Modern Healthcare. Yet, there’s a major obstacle: people are hesitant to share information about the non-medical challenges they face with their healthcare providers — and understandably so. It’s difficult to open up to even friends or family about some of these challenges, let alone a doctor or nurse. Trust is key, and there's too little of it in so much of healthcare. That's why a community-based approach to health care — making sure people who are rooted in communities are engaging and interacting with patients — is so important. Familiarity breeds trust, and trust is the only way to really learn about the whole person and then help them get healthy. #valuebasedcare #communityhealthcare #provders #Walgreens
