This taken from an internal communication here a Elenchos Health referring to one of Care managers. During a routine follow-up, Hannah was informed by a patient of difficulty breathing. She immediately recognized the gravity of the situation and directed the patient and her daughter to call 911, which they initially refused. Undeterred, Hannah promptly reached out to the patient’s office to notify them of the critical situation. Following Elenchos protocol for urgent situations, she took the extra step to ensure the patient was not left unattended to. Hannah’s persistence in advocating for the patient was critical. She called back to emphasize the urgency of seeking emergency care, which ultimately led the patient to the E.R. The following day, Hannah continued to show her exemplary care by checking the patient’s EMR and confirming that the patient had indeed followed her advice, resulting in the necessary medical attention that was crucial for the patient’s health. This situation is a powerful reminder of the impact our care can have on patients’ lives. Hannah’s actions demonstrate an extraordinary level of dedication and care.
