RN Navigator Details
Essential Job Duties
Acts as an advocate for the patient in assisting patients to reach optimal function and health in an appropriate environment that would support their needs
Applies that process of assessment, planning, implementation, monitoring, evaluation and coordination of care to meet the patient's health care needs across the continuum in collaboration with the health team members to ensure safe, quality and cost-effective care.
Collaborates with patient/family in establishing mutual goals based on the patient's needs or problems.
Collaborates with health care team to coordinate care across the continuum to assure appropriate utilization of clinical and community resources.
Explores patient's understanding and knowledge of current health status.
Collaborates and helps coordinate services with the Care Teams throughout the health system as needed.
Serves as part of the Accountable Care Organization Care Management team to maximize effective use of resources and manage cost.
Qualifications
Must have active IN RN license
Must also have IL RN license or be eligible to apply for IL RN licensure
Must have graduated from an accredited nursing program
Must have active BLS certification
Healthcare experience required
License/Certification
Must have active IN RN license
