We are seeking a dedicated Care Coordinator, Behavioral Health to join our dynamic team. The Care Coordinator, Behavioral Health will play a pivotal role in ensuring the seamless coordination and integration of behavioral health services within our multidisciplinary care teams. This role will involve working closely with internal and external stakeholders to enhance member health outcomes and promote holistic care approaches.
Your Impact
Work effectively and diplomatically within multidisciplinary care teams to integrate behavioral health services into overall member care plans
Provide face-to-face services to members in various settings (e.g., in-home visits, synchronized video, community-based locations) to foster continuous and integrated relationships with their care teams
Identify and minimize risk factors impacting member health outcomes through comprehensive assessments of physical health conditions, behavioral health issues, mental status, social supports, and social determinants of health needs
Evaluate member motivation and readiness for change to ensure that care and service engagement align with member needs
Develop individualized, person-centered, and trauma-informed care plans based on assessment information, utilizing motivational interviewing techniques
Monitor and review member care plans regularly to assess progress, revise goals, and update interventions as necessary
Maintain accurate and current documentation of all care management activities to facilitate effective care coordination
Ensure a closed-loop system for behavioral health referrals to promote high-quality care continuity
Provide brief behavioral interventions using evidence-based techniques (e.g., behavioral activation, problem-solving treatment, motivational interviewing)
Facilitate member transitions through the continuum of behavioral health care, ensuring placements align with best practices and medical necessity
Educate members on their rights, responsibilities, and consent requirements for release of protected health information necessary for coordinated care
Coordinate emergency assistance for members, including communication, paperwork completion, and submission of necessary requests
Conduct community outreach and education on Umpqua Health’s Behavioral Health Care Coordination program
Perform other duties as assigned by management needed to help drive to our Vision, fulfill our Mission, and abide by our Organization’s Values
Your Credentials
Bachelor’s degree in Social Work, Psychology, Counseling, or related field required; Master’s degree preferred
Previous experience in behavioral health coordination, care management, or related field preferred
Strong understanding of trauma-informed care principles and motivational interviewing techniques
Excellent communication, interpersonal, and organizational skills
Ability to work collaboratively within multidisciplinary teams and across diverse populations
Proficiency in maintaining detailed and accurate documentation
Commitment to promoting health equity and culturally responsive care practices
