There’s a category we believe is getting too little attention when it comes to AI in healthcare: AI Clinical Documentation Integrity (CDI) While AI ambient scribes and autonomous medical coding solutions have gotten most of the headlines, we’d argue AI CDI is equally impactful when it comes to quality care and accurate billing. For those not familiar, CDI ensures that clinical documentation accurately reflects the patient’s medical condition and the provided care. Historically, CDI was a manual slog, with dedicated CDI specialists, nurses, and physicians painstakingly reviewing patient charts to ensure documentation supported the care delivered. Today, AI is reshaping this landscape. While it’s still early days, we’re seeing three primary approaches to applying AI when it comes to CDI: 1️⃣ CDI + ambient scribing—an AI ambient scribe and AI CDI assistant operate simultaneously, generating comprehensive notes and surfacing codes for physicians to review at the point of care. (Example: Ambience Healthcare’s AutoCDI) 2️⃣ CDI via real-time diagnosis support—diagnoses are suggested at the point of care based on clinical data in the EHR, with auto-generated documentation and audit trails. (Example: Regard) 3️⃣ CDI via an AI-driven post-coding / pre-billing review—the entire patient record is ingested and analyzed against what is coded, surfacing suggestions for revenue and quality opportunities. (Example: SmarterDx) To date, AI in healthcare has focused primarily on administrative use cases. With the increasing adoption of AI CDI solutions, we’re now taking a step closer to clinical workflows. As clinicians get used to AI-suggested diagnoses in the CDI context, it will be interesting to see if this increases their comfort with AI playing a role in clinical care. UnitedHealth testing 500 AI use cases UnitedHealth Group is running around 500 use-case applications for AI across the organization, CEO Andrew Witty told investors. Speaking at a conference on May 29, Mr. Witty said some of the company's AI efforts are "low-hanging" opportunities aimed at speeding up administrative processes. "We have a very strict internal and external government board around all of what we do in AI," Mr. Witty said. "We're focused on making sure there is a human in the loop anytime you get close to what you would characterize as clinical." The Major Question for all-"Does anybody trust anything Optum or UHC ever says?" Nothing gets in the way of profits and payouts to its C suite or shareholders. Somewhat predictably, the headlines of Epic's UGM were mostly focused on AI. In my mind, the ones to focus on got buried as footnotes. Products leveraging Epic's position as the dominant inpatient EHR and selling differentiated products as a wedge into new markets (mostly under Health Grid) should frighten other healthcare vendors more than the AI arms race (which they'll largely win as the default option): - Payer Platform: A Health Grid product for payers. This was decently well covered, but that's because it's a mature product at this point. A lot of outlets covered it as if it was net new, but it was launched in 2019. The most noteworthy piece is Judy's quote saying they’re investigating submitting claims directly through Epic so you don’t need a clearinghouse - this represents a threat to clearinghouses and could represent the expansion from Payer Platform as a beachhead to attack incumbent SaaS, claims processing systems. - Epic Aura (labs): This is a Health Grid product for labs. This was launched previously. I don't see any announcements, but this is a beachhead for Epic to sell to labs that have or built their own LIS, allowing them to eventually replace it over time (or at least have all major labs as customers). This threatens existing LISes even more than Epic's LIS product, Beaker. They also announced a planned Blood Bank module, which increases their appeal as an all-encompassing LIS in this push. - Epic Aura (medical devices): A new sub-category of Aura (and Health Grid) for expanding into medical devices. I was waiting to see if they'd launch a separate category for medical devices or reuse Aura. First devices are cardiac arrhythmia and CGM. This threatens businesses like Redox to some degree. - Per Healthcare IT today, "Judy said they’re working on better surgical device manufacture planning leveraging the Epic schedule." This sounds like further expansion of Aura/Health Grid to surgical supply manufacturers. - I was surprised to not hear any updates about Discovery, Epic's Health Grid program for pharma. I'm curious what shape that will take. - I thought the use of the state of Washington + the Garden Plot model is a smart distribution play. Garden Plot has existed since at least 2022, but using a state for distribution to get to new markets that Epic struggles to sell to (small clinics, behavioral health clinics, correctional medical facilities and tribal facilities) is again another example of their expansionary mode.
