Recently the FDA Requests Class-Wide Boxed Warning for CAR T-Cell Agents Regarding Secondary T-Cell Malignancy Risk, in this article the researchers examined the FDA Adverse Event Reporting System (FAERS) to assess the occurrence of secondary primary malignancies (SPMs) linked to CART treatment. They analyzed 536 reports, with axicabtagene ciloleucel and tisagenlecleucel being the most commonly cited. Leukemia, accounting for 62.1% (333/536) of cases, represented 2.7% of all CAR T reports. The predominant SPMs were leukemia, encompassing myelodysplastic syndromes, acute myeloid leukemia, and T-cell lymphocytic leukemia. Skin neoplasms, including non-melanoma and melanoma, ranked as the second most frequent SPM, followed by hematopoietic neoplasms, nervous system tumors, and respiratory neoplasms. Analysis revealed a higher reporting odds ratio (ROR) for myelodysplastic syndromes with axicabtagene ciloleucel, tisagenlecleucel, idecabtagene vicleucel, and ciltacabtagene autoleucel. Tisagenlecleucel and ciltacabtagene autoleucel were linked to a higher ROR for acute myeloid leukemia. The FDA reported 22 cases of T-cell malignancies associated with five of the six CAR T products. Genetic sequencing identified the CAR transgene in the malignant clones of three cases. Nineteen cases of T-cell malignancies were identified, two of which were linked to ciltacabtagene autoleucel and lisocabtagene maraleucel. However, evidence regarding whether CAR integration drove malignant transformation was inconclusive.
