    Multifidus atrophy is a measurable and objective MRI biomarker associated with the #Chiropractic subluxation. In this case series, patients exhibiting disc herniation, degeneration, radiculopathy, and associated symptoms received SMT with specific exercise. A decrease in multifidus atrophy was observed on MRI. The restoration of segmental mobility using SMT is vital to spinal neuromusculoskeletal health.     A 63-year-old who experiences chronic neck pain with intermittent exacerbation of upper extremity radicular symptoms. He reports no motor deficits, slight alteration of sensation along the C5 and C6 dermatomes, 1/5 DTRs, negative Hoffman/Tromner signs, negative bladder/bowel/saddle issues, negative lower extremity or gait abnormalities. No other issues or co-morbidities are present. Chiropractic HVLA-spinal adjustments in ranges of motion that do not peripheralize pain and intermittent axial decompression provide relief. NSAIDs provide relief during acute exacerbations. Recent MRI images are attached. The patient is concerned about his progression over the next 5, 10, 20 years. He is concerned about his spinal cord integrity and the likely progression of degenerative cervical myelopathy. His neurosurgeon and orthopedic spine surgeon, whom he trusts implicitly, recommend no surgery at this time. However, the patient is concerned about delaying decompression and waiting for spinal cord and CNS changes to trigger a surgical intervention. Should this patient undergo surgical decompression with fusion or joint arthroplasty to avoid progressive and permanent neurodegeneration?

    Attaining better outcomes requires that we speak in a manner our patients clearly understand. Informed consent is a process. It is a meaningful, ongoing discussion between patient and doctor. When done well, it answers important questions, quells fear, and provides patients with insight into expectations. Last Friday, Dr. Demetrious saw a patient with acute LBP of approximately two weeks' duration. He suspected L5/S1 facet synovitis with localized paraspinal spasm. He informed her of his diagnosis and recommendation for Chiropractic HVLA – spinal adjustment. He reviewed his differential diagnosis, home care, activities of daily living, risks/benefits, possible need for imaging, and alternative care options. Dr. Demetrious advised her of expectations following his care. He explained that she might feel immediate relief after the adjustment but would likely experience a return of acute pain that evening and possibly the next day. He advised that sharp pain would progressively subside. He had her repeat his recommendations. She consented, and he adjusted her in pain-free ranges of motion. As she rose from the table, she reported immediate relief. A couple of days later, she returned for follow-up care. Overall, she was about 60% better. She thanked Dr. Demetrious for warning her about the return of acute pain she experienced that evening and the next day. She was grateful that he took the time to inform her of her likely progression. She trusted Dr. Demetrious because he verbalized likely expectations. She was appreciative of his preemptive efforts to allay her fears and provide her with relief.     In practice, chiropractors observe substantial anecdotal relief of neurologic complaints. The measurement of SEP N30 potential may be a viable biomarker to assess the benefits of #Chiropractic for neurodegenerative conditions. Our colleague, Dr. Robert Trager and co-authors reported a single session of chiropractic spinal adjustment reduced SEP N30 potential and enhancement in connectivity within the DMN at the alpha, beta, and theta frequency bands in individuals with Alzheimer's disease.     We are most grateful for those doctors who attended our Grand Rounds presentation last evening. We enjoy spending time with our colleagues in a productive manner.
