The following is a suggested list of information and documents you should have prepared: • Wills or trusts • Durable powers of attorney • Healthcare powers of attorney • Data on funeral planning—pre-need arrangements, burial or cremation contracts • Written instructions on specific wishes for wakes, funerals or memorial services • List of all property, including bank accounts, investment and retirement accounts, real estate, annuities, etc. (including statements with account numbers and location of titles) • List of all debt—mortgages, home equity, auto, boat, education loans and credit cards • List of all credit or debit card account numbers, security numbers and PINs • Information on safe deposit boxes, including bank name, location of key and box number • List of “important” or valuable personal property • List of all income sources—Social Security, pensions, annuities, IRAs, etc. • List of all insurance policies • List of all employers worked for, unions joined or military service • List of all advisors, including accountants, attorneys, brokers and insurance agents • Personal telephone book (possibly holiday card list) • Location of tax returns • User IDs and passwords to computers, websites and any financial software Planned Giving Roundtable of Southeast Michigan

About us
The mission of the Planned Giving Roundtable of Southeast Michigan (PGRTSEM) is to increase the use of planned gifts for support of charitable organizations through the dissemination of information regarding the advantages and techniques of planned giving. Membership is made up of professionals in the legal, financial, and allied fields as well as fundraising professionals. Through educational and information exchange opportunities and cooperative networking, members offer an organized voice that raises the consciousness of donors and their advisors about planned gift methods. The PGRTSEM is a member council of the Partnership for Philanthropic Planning (PPP), the leading national organization dedicated to increased the quality and quantity of planned gifts through education, communication, and the promotion of a code of ethics as stated in "The Model Standards of Practice for the Charitable Gift Planner." LEAVE A LEGACY(R) is a national public awareness campaign, sponsored locally by PGRTSEM, to promote planned giving.
