This past week, two members from our team at Partners Health Plan, Annrose Bacani, MHA and Karleen Mirador Haines, MPS, presented at the 9th Annual Fiscal Intermediary/Self-Direction Summit hosted by the New York Alliance for Inclusion and Innovation. It was another successful event focusing on education, networking, and empowerment for the IDD community. Our team presented on the importance of staying in your best health to maintain independence and connection to your community through a whole-person care model, access to health and behavioral services, and supportive care management.
