At Baystate Medical Center since 2005, Karen serves as the Manager of Case Management, overseeing the Case Management Team, Utilization Management, and Appeals and Denials. Karen is a Certified Case Manager and a member of ACMA, sitting on the ACMA MA Chapter Membership Committee. Justin Krawitt MD, MHCDS, CHCQM Dr. Krawitt is an Assistant Professor of Medicine at the Geisel School of Medicine at Dartmouth College and serves as the System Medical Director of Utilization and Clinical Documentation at Dartmouth Health. He is also the President and Founder of Hanover Physician Advisors, a consulting firm specializing in Utilization, CDI, and Revenue Cycle, as well as the President of the NH-VT Chapter of the Society of Hospital Medicine. Dr. Krawitt trained in internal medicine at Dartmouth-Hitchcock Medical Center. He obtained his MD at the University of Louisville School of Medicine and received his bachelor’s degree from the University of Vermont. He is a practicing Hospitalist and his clinical interests include COPD/CHF GDMT and Point of Care Ultrasound. Thaddeus (Tad) Comeau, RN, CCM Tad Comeau, RN, CCM is currently the Vice President of Care Management for Senior Care Options (SCO), Duals and Public Plans at Point32Health. In his role Tad is responsible for providing leadership over the clinical programs, and processes that support the care and disease management of our members for the SCO and Public Plans lines of business. Prior to joining Point32Health, Tad has worked in the field of case management for over 12 years in both staff and director roles. He most recently served as the Director of Complex Care Management at Boston Medical Center for 5 years where he had oversight of the care management program of the BMC Mercy Alliance ACO. Prior to entering the field of case management Tad spent several years working in community health and home care. He worked as staff RN and supervisor for VNA Care Network for many years as his introduction to home health. He then worked for 5 years as a home infusion RN and clinical liaison for Critical Care Systems home infusion in Worcester MA. In 2005 when noticing an underserved area for home care in the state, Tad opened his own private home care agency, Heritage Home Health, which he grew and ran successfully until it’s sale in 2011. Tad’s early career in nursing was spent in both acute rehabilitation and trauma ICU. Tad holds a degree in Social Work form Assumption College and went on to earn is RN degree and holds a certification in case management. He is currently enrolled in MBA Health Care Administration at Fitchburg State University. In his free time Tad enjoys spending time with his wife and 4 children. Symposium Speaker Spotlight! Our Keynote Speaker, Ryan Thompson, MD, MPH Dr. Ryan Thompson is a general internist at the Massachusetts General Hospital (MGH). He sees inpatients as part of the MGH Complex Care Service and outpatients at MGH Internal Medicine Associates; he also attends periodically on the Bigelow Teaching and Home Hospital Services. Dr. Thompson serves as the MGH Medical Director of Care Continuum and as Associate Medical Director of the MGH Capacity Coordination Center. He helps organize and lead MGH initiatives supporting patient throughput and outflow and collaborations with post-acute and home health care. His primary clinical interest is innovative care redesign for the most complex and vulnerable patients that prioritizes continuity and trust. He earned his medical degree from the University of Utah and completed his internal medicine residency training at MGH, followed by a year as chief resident. Debbie Costello RN MSM Debbie Costello, RN, MSM, is a registered nurse and executive with over 30 years in home health and hospice, and a longstanding leader in the post-acute space. She currently serves as Chief Operating Officer for Beth Israel Lahey Health at Home which is comprised of home health, palliative care and hospice services, and is responsible for leading strategic initiatives, promoting quality care, and overseeing financial performance. She engages with leaders across the healthcare system to coordinate and improve care delivery. Recently served on the board of directors for the Home Care Alliance of Massachusetts for six years and is a strong advocate for receiving care in the home. Prior to joining BILH at Home in 2013, she was an independent consultant and also held executive leadership roles with responsibility for overseeing operations, quality program development and cross-setting transitions in care work. Laura Smith, MD Laura Smith, MD is the Senior Medical Director of Clinical Operations for Brigham and Women’s Hospital in the Office of the Chief Medical Officer, and a practicing hospitalist. She joined Brigham in 2012 as a resident in Internal Medicine and joined the faculty in 2015 in the Division of Hospital Medicine in the Department of Medicine. In her role as Senior Medical Director for Clinical Operations, Dr. Smith is responsible for shaping and implementing Brigham and Women’s Hospital strategies that involve clinical and operational collaboration and integration between the hospital administration and the clinical departments or cross-cutting multidisciplinary or cross-department initiatives that serve the Brigham and Women’s Hospital mission. Anuj Dalal, MD, SFHM Anuj K Dalal, MD, SFHM is an Associate Physician in the Hospital Medicine Unit, Division of General Medicine at Brigham & Women’s Hospital and Associate Director, Harvard-Brigham Research Fellowship in Hospital Medicine. He is an Associate Professor at Harvard Medical School. His body of work has focused on designing, developing, implementing, and evaluating novel, interoperable digital health technologies to improve quality and safety in acute care. Dr. Dalal is nationally recognized in his field and his research has been published extensively at the intersection of informatics, quality, and safety. His work has been funded by AHRQ, CRICO, PCORI, and Gordon and Betty Moore Foundation. Dr. Dalal led a 4-year AHRQ-funded Patient Safety Learning Laboratory during which his team developed an approach for investigating diagnostic errors by analyzing the diagnostic process and pilot testing interventions to improve diagnostic safety in acute care. He currently leads an AHRQ R01 that uses novel predictive approaches and user-centered design principles to design tools to engage patients in real-time symptom monitoring using ePROs during care transitions.
