Scaffold failures frequently result in large financial losses and construction/remediation project delays. Common factors which may contribute to scaffold failures include: • Wind load not being captured correctly • Snow load not being considered in the design of the temporary roof structure or canopy atop the scaffolding • Improper sizing of fasteners used to secure the scaffold to the main structure • Insufficient due diligence when verifying construction of the scaffold and roof/canopy • Omission or mis-installation of scaffold locking pins • Undersized, missing, or improperly positioned sole board • Out-of-plumb adjustable base plate • Deviation from construction details In this article, J.S. Held forensic engineer Kenny Yip provides deeper insights on the common causes of scaffold failures, as well as preventative measures to mitigate the risk of personal injury, property damage, and project delays. As artificial intelligence (AI) becomes more entrenched in business operations, the risk of cyberattacks increases, which can lead to reputational damage, litigation, greater regulatory scrutiny, financial penalties, and higher compliance costs. A Chief Information Security Officer (CISO) is responsible for mitigating and responding to cyber incidents, but not all companies have a full-time CISO. In this article, Kevin Gorsline discusses the role and benefits of a Virtual CISO in filling that resource gap, delivering expert cyber security advisory, and reducing cyber risk.
