Differences between the European Approach to Value Based Care and the American Approach You’ve heard Juliette Price outline some similarities observed at ICHOM 24, but what about the differences? Here are three key divergences: -US is hyper focused on cost control; others focused on quality: While cost and quality are inherently linked (both sides of the Atlantic will agree), the European conversation focuses much more on quality improvement and how to imbed QI and VBC -Including the patient voice: certainly the US is moving to do more about collecting patient experience data (STAR ratings, NPS, HCAHPS, etc), but our European colleagues are letting it play a much bigger role, especially in the transition to VBC -Government’s role: our cross-Atlantic partners are saying the quiet part out loud—the need for government (at multiple levels) to demand value based health care and set the framework for the transition is a key part of the work ahead. 👷⚒️🪚Common Challenges⛓️💥⁉️ As countries around the world implement VBC, there are some clear, common infrastructure challenges that we’re all facing—let’s dig in: -Measures, data capture, data use: International Consortium for Health Outcomes Measurement (ICHOM) is an invaluable resource as countries begin to commit to measuring patient outcomes, not just inputs. But even with the right measures in place, collecting the right data points, organizing it, and most importantly sharing that data with clinical providers in order to get to improvement and practice change is a huge infrastructure shift from FFS. -Adaptability of VBC models: Many countries have started their journey to value based health care in speciality areas, focusing on what we’d refer to as bundles or choosing to focus on specific disease groups. What’s challenging is the larger (in)adaptability of these models across different areas of the health care ecosystem, such as primary care. -VBC as a deep cultural shift: So many countries are struggling with the long-term fiscal and moral implications of perpetuating the current fee-for-service system—but the shift to value is not just a technical exercise, it’s deep culture change.
