A technology-enabled healthcare provider delivering care to those who don’t have it. Starting in rural America.
About us
Homeward is rearchitecting the delivery of health and care for rural Americans by connecting payers, providers, and community organizations through partnership. By taking full financial accountability for clinical outcomes and the total cost of care across rural counties, Homeward aligns incentives and strengthens local healthcare ecosystems. The Homeward Navigation™ platform uses advanced analytics to connect members to the right care and local resources that address social determinants of health and improve holistic health outcomes. Since many rural communities lack adequate clinical capacity, Homeward employs care teams that supplement local practices and reach people who cannot access care otherwise. As a public benefit corporation and B Corp™ certified company, Homeward’s mission and business model are aligned to address the healthcare, economic, and demographic challenges that make it harder for rural Americans to stay healthy.

Last week, Homeward Director of Clinical Strategy Dr Sophy Hu BA Hons MBBS (MD) represented Homeward at a roundtable discussion with the U.S. Department of Health and Human Services (HHS) Secretary Becerra to discuss how the federal government is investing and growing access to care using #AI tools. We appreciate the opportunity to engage in these important conversations around leveraging technology to scale healthcare access to underserved areas, including #rural communities.

Rural health has long been a challenge in America, where access and workforce shortages drive outcome disparities. Homeward Co-Founder and President Amar Kendale will join Nashville Health Care Council's #Sessions24 Conference to discuss the critical importance of incentive alignment in #rural care delivery, the role of technology in going beyond telehealth solutions, and more to highlight innovative solutions tackling one of the nation's greatest healthcare needs.

This past weekend, the Homeward team traveled near and far to participate in the annual 100-mile Breakthrough T1D (formerly Juvenile Diabetes Research Foundation) Ride to raise awareness of and money for Type 1 Diabetes research. Cycling through the picturesque landscapes of Wisconsin, Iowa, and my home state of Minnesota, I was moved by the familiar and new faces united for this meaningful cause. This annual event holds a special place in my heart, and I am deeply grateful to Breakthrough T1D for their commitment to finding a cure, and for the Homeward team who supported through donating, volunteering, riding and cheering on Team Homeward.

This past weekend, Homeward colleagues traveled to La Crosse, Wisconsin biking up to 100 miles across the rural landscapes of Wisconsin, Iowa and Minnesota in support of Breakthrough T1D (formerly Juvenile Diabetes Research Foundation) and crucial research that changes the lives of millions affected by Type 1 Diabetes (T1D). Together, Homeward raised over $70,000 for T1 diabetes research. Thank you to everyone who supported us on this journey!

A recent study from the University of Minnesota Rural Health Research Center found that gaps in access to healthcare can exacerbate risks to the well being of older #rural residents caused by social isolation, injuries, or food insecurity. As the U.S. population ages, many Americans age 65 or older are making their homes in rural communities, and the need for integrated – and sustainable – healthcare solutions has become even more critical. Homeward’s approach and partnership with health plans and health systems in rural communities directly addresses these issues, working closely with providers to connect members to appropriate care and local resources. https://lnkd.in/eFJz23U5
