About us
The Centers for Medicare & Medicaid Services (CMS), a federal agency within the U.S. Department of Health and Human Services, is one of the largest purchasers of health care in the world. It’s #NationalPrimaryCareAwarenessWeek. #PrimaryCare is the foundation of a high-performing health system. Primary care doctors often have long-term care relationships with patients and their caregivers, making them more alert to health changes. They may also serve as an “entry point” to health care for a patient without a clear diagnosis. The CMS Innovation Center has invested in several models (pilot programs) to strengthen primary care. Check out this infographic: go.cms.gov/pc-graphic. For more information about primary care and how it’s important for your health visit the Primary Care Key Concept: go.cms.gov/pc.

Did you know nearly 90% of Americans struggle with health literacy, according to the National Library of Medicine (NLM)? 📚 People with low health literacy are more likely to have poor health outcomes due to trouble understanding medication instructions, difficulty managing chronic diseases, or skipping preventive services. #Valuebasedcare can better support children to grow up to be healthy adults. That’s the focus of CMS’ Integrated Care for Kids (InCK) Model (pilot program). For a Chicago-based, bilingual social service provider who is part of InCK, it’s about connecting families to resources, both in and outside of the doctor’s office. She helps families in crises access programs to meet their individual needs—whether it’s connecting them to a food bank, helping get their child a new wheelchair, translating school forms for extra support, and more. #WhatDoYouValue Watch the full video (URL) and/or read the full story: https://go.cms.gov/3MWtQn4 and Spanish: https://go.cms.gov/3TSWzwR.
