Excellent post from @yourheartdoc! Coronary plaque rupture. . This visual shows what is happening within a coronary artery during plaque rupture. This is the pathophysiologic mechanism of a myocardial infarction - or simply put - a heart attack. This is the cross section of a coronary artery. . 1️⃣ Over the course of years, atherosclerotic plaque builds up within the lumen of the artery. A major contributor is lipoprotein deposition via high LDL cholesterol levels. This is complemented by inflammation, fibrosis, and calcification. . 2️⃣ Eventually this plaque can grow to be bulky and complex and be highly inflammatory. These "vulnerable" plaques are often thin-capped atheromas encasing a lipid rich and very necrotic material. . 3️⃣ At some point one of these vulnerable plaques can rupture their thin cap, exposing all that yucky "schmutz" to the blood stream. Think of this as an inflamed white head zit that grows and eventually pops! . 4️⃣ The body reacts to this schmutz, initiating the platelet aggregation and clotting cascade. It's like the body reacting to a cut. What does it do? Wants to make a scab to plug it up - which in turn shuts down the coronary artery. Oops! . . 💔Knowing this, one can understand why we use statins, diet/lifestyle, smoking cessation, diabetic and BP control to slow down the early stages of plaque development. And why we use anti-platelet agents, heparin, nitro, and revascularization with thrombolytics or stenting in the acute presentations of a rupture. . .
