We’re excited to introduce you to our 2025 class of new partner organizations: Graves Gilbert Clinic in Kentucky, Mankato Clinic in Minnesota, Springfield Clinic in Illinois, and Twin Cities Network in Minnesota as well as an independent, multi-specialty practice in North Carolina that will be named at a later date. Last week, four of our physician partners from New York and Pennsylvania joined us to meet with members of Congress in Washington, D.C. Our conversations focused on the need for continued support of value-based care models like ACO REACH and the Medicare Shared Savings Program (MSSP), which are transforming care for patients and primary care physicians. Drs. The Centers for Medicare & Medicaid Services (CMS) recently spotlighted our physician partner, Dr. Shari Rozen from Preferred Primary Care Physicians as part of its value-based care spotlight series. “We’re not trying to see as many patients as we can cram in a day,” Dr. Rozen said. “We’re slowing down, we’re keeping patients out of the hospital, we’re doing what we’re supposed to do – focusing on providing better care.” Dr. Rozen shared her story on being part of a full-risk Accountable Care Organization (ACO), and how it has enabled her and her practice to offer personalized services that better meet the unique needs of patients. For one family doctor, it’s collaborating with social workers and case managers, made available through an Innovation Center pilot program, who work with her to address the unique needs of patients and reduce avoidable hospitalizations. A patient with chronic obstructive pulmonary disease told her that because of this extra support he no longer feels like he must go to the emergency room every time he’s short of breath.
