Patients’ non-medical needs impact their health and contribute to health inequities. Hospitals and health systems are identifying best practices for social needs screening, and collecting and connecting patients with resources; and health care professionals have an important role to play in that process. Meeting patients’ social needs is a collaborative effort, which requires engagement of various professions from across the health care spectrum and community. This virtual workshop highlights the power of robust, interdisciplinary care teams in addressing social needs. Learn how Regional One Health and NewYork-Presbyterian Hospital improved health outcomes by leveraging specialized skills of care team members to identify and address the social needs of their patients.
