Amita Health's Social Worker in the Library Chicago project, which embeds a Licensed Clinical Social Worker at two library branches to improve the quality of life for individuals who are experiencing homelessness, mental illness and other complex needs in the Uptown community of Chicago. In the first seven months, the social worker interacted with close to 440 patrons, in which more than 250 were referred to programs and services on social determinants of health.
