Robotic Surgery in Oncology & Urology

Objectives: The  current  standard  method  for  diagnosing  bladder  outlet  obstruction  in  patients  with  Lower  Urinary  Tract  Symptoms  (LUTS) is a pressure-flow study. Electronic perineal auscultation, i.e. recording sound with a contact microphone behind the scrotum during voiding,  might  be  an  easy-to-use  non-invasive  alternative.  Of  major  importance  for  the  clinical  applicability  of  perineal  auscultation  is  the  quality of the sound recording. Methods: In  a  clinical  population  of  74  male  LUTS  patients  we  studied  two  measures  for  assessing  the  quality  of  electronic  perineal  auscultation: a Normalized Correlation Coefficient (NCC) and Signal-To-Noise Ratio (SNR). We compared these measures with the visual assessment  of  the  electronic  auscultatory  signal  by  three  experienced  observers.  The  agreement  between  the  experienced  observers  was  expressed using the Weighted Kappa statistic. Both quality measures were compared to the combined visual assessments using the Kruskal Wallis test and the usefulness was evaluated using ROC-analysis. Results: The Weighted Kappa values between the three observers were 0.73, 0.66 and 0.76 (p<.05).For recordings classified as ‘good’ by the observers, the NCC was significantly higher than for the ‘medium’ and ‘poor’ quality recordings (p<.05). The areas under the ROC curve were 0.84 and 0.66 (both significant, p<.05) for NCC and SNR, respectively. Conclusion: We developed quality criteria to assess the quality of electronic perineal auscultation in patients with LUTS. These quality measures  can  be  used  to  select  auscultatory  measurement  signals  that  are  suitable  for  further  analysis  and  for  improving  the  perineal  auscultation setup. In  men  the  prostate  generally  increases  in  size  with  age,  most  often as a result of Benign Prostatic Enlargement (BPE). The latter may  lead  to  Bladder  Outlet  Obstruction  (BOO).  Men  with  BOO  generally have Lower Urinary Tract Symptoms (LUTS), including a weak urinary stream, frequent voiding (also nocturnal) and residual urine in the bladder after voiding. The current standard method for diagnosing BOO is a pressure-flow study (PFS) which is part of a (video) urodynamic examination (VUDE).

The aim of the study was to test the post RT PCR High-Resolution Melting Curve assay in clinical specimens from patients in Greece, as a rapid screening method for the detection of the genetic variation of SARS-CοV-2. Methods: Out of 620 clinical RNA isolates 60 were affirmed positive by two methods, a commercial kit and a home-made method developed at our Laboratory in the University of West Attica. With an indicative seropositivity prevalence of 9,7%, we further analyzed the 50% of the positive specimens with RT-PCR combined with a highresolution melting (HRM) curve analysis for the simultaneous detection of variations in E, S2, N1 and RdRp genes of SARS-CoV-2. Results: Tested samples comprised with 10.000 to 1.000 copy numbers of E and RdRp amplicons with 60 and 66,7% for the S2 and N1 genes respectively. It was shown that HRM assay of the positive samples classified with the highest variation the E gene (66.7%) within three clusters. N1 gene amplicons exhibited 2 clusters with a presence in the 50% of the specimens in each cluster. As for the S2 gene it was not revealed any variation, forming only one cluster. Similar results were shown for RdRp gene, where the 95% of amplicon belonged to the same cluster. Conclusions: HRM assay can be a useful tool for the rapid screening of large numbers of patients, providing an early warning for the emergence and spread of regional variants and strains of SARS-CoV-2, which may bear clinical and epidemiological concern.

Today, radical prostatectomy is performed by open retropubic and perineal access, by conventional laparoscopic and robotic ap-proaches. Robotic surgery has been developed to increase oper-ative precision, decrease the learning curve and thereby increase the applicability of minimally invasive laparoscopic techniques. It provides the best ergonomics for the laparoscopic technique, resulting in a shorter learning curve of a well-standardised proce-dure such as radical prostatectomy. In the United States, &gt;70% of radical prostatectomies are performed using the robotic surgical system. The introduction of surgical robot to assist laparoscopic radical prostatectomy has helped to evolve the laparoscopic skills and improve surgical maneuverability with enhanced visual systems and the multi-axis articulating instruments. Abstract : Objective:The purpose of this study was to analyze Articles of nephron sparing surgery (NSS) to radical nephrectomy (RN) for Localized Renal Cell Carcinoma (RCC), of two different surgical by means of the method of meta analysis, in order to provide evidence for clinical treatment of Localized Renal Cell Carcinoma. Methods:Articles including randomized controlled trials (RCTs) studies around the world about nephron sparing surgery (NSS) to radical nephrectomy (RN) for Localized Renal Cell Carcinoma (RCC), were searched by Pubmed, Ovid, CENTRAL(CochraneLibrary), EMBASE, EI, CBMdisc, VIP,WanFangData; studies were chosen at last for Meta-analysis. Results:In accordance with the inclusion criteria selected There were 16 RCTs studies of (NSS) versus RN for RCC included in this Meta-analysis.A total of 20972 patients from 16 studies were included,of which NSS group of 4952 and a RN group of 16020. After 3 years of cancer specific mortality relative risk (RR) RR=0.88,95%CI [0.70 1.12], P=0.30; after 5 years of cancer specific mortality relative risk (RR) RR=1.07,95%CI [0.75 1.51], P=0.71;After 7 years of cancer specific mortality relative risk (RR) RR=0.88,95%CI [0.77 1.01], P=0.07;the postoperative recurrence of tumor is the relative risk (RR) RR=1.03,95%CI [0.69 1.55], P=0.88; postoperative tumor metastasis comparison of relative risk was (RR) RR=0.99,95%CI [0.77 1.27], P=0.92; the occurrence of postoperative bleeding is the relative risk (RR) RR=1.58,95%CI [1.03 2.42], P=0.04; There were statistical significance;the occurrence of postoperative renal failure cases compared to the relative risk (RR) RR=0.97,95%CI [0.68 1.41], P=0.89; the occurrence of postoperative mortality compared to the relative risk( RR) RR=0.92,95%CI [0.68 1.23], P=0.56. Conclusions:There is no significant differences between NSS and RN with regard to five year cancer related mortality.Tumor recurrence and metastasis. NSS could maximally preserve the function of the remaining nephron and reduce the risk for kidney function failure.Although RN is superior in the prevention of prospective complications,NSS is an effective and reliable treatment for localized renal cell carcinoma. Introduction: Cancer of breast is one of the greatest problem which india is facing.There are many studies done to study association between role of diet and blood lipids.Moreoverthere are multiple factors both, environmental and genetic which do play a role in evolution of carcinoma breast. The present study was undertaken to find if any association exists between alterations in lipid profile in carcinoma breast of local population. Material and methods: A total of ninety one patients with various stages of breast cancer who were being treated at the department of surgery, during Nov 2013 to June 2015 were recruited in the present study. One hundred healthy volunteers were included as controls. Patients were categorized according to age and body mass index with their respective controls. Serum lipid profile was performed in all the cases and control groups using fasting blood samples. Total cholesterol , HDL-cholesterol and Triglycerides were measured by standard kit methods and LDL cholesterol was calculated using Fried Wald's formula. Results: Serum Total Cholesterol, HDL Cholesterol and LDL Cholesterol have been observed to be significantly elevated in controls when compared with those of cancer patients (p=0.05, 0.007 and 0.011 respectively). Triglycerides did not show any significant alteration between that of the cases studied and the controls (p=0.30), though the value was more in controls than in cases. Conclusion: This preliminary study has shown a significant alteration in serum lipid profile of breast cancer patients and healthy group in the local female population. The interesting observation of low levels of HDL cholesterol and high levels of LDLcholesterol, Total Cholesterol and Triglycerides in controls rather than in cases, needs further evaluation by extending the investigation further on and also extending the study on larger study group. This result could be partly explained by the fact that low cholesterol in cases is the result of the effect of cancer rather than not on the cause of cancer.

The total urinary exenteration and extended lymphadenectomy was successfully performed with robot- assisted surgery in this unique case where the patient was rendered anephric at the end of the operation. We discuss the operative steps and the techniques performed to opti- mize the oncological results of robot-assisted surgery for invasive urothelial carcinoma, while attempting to preserve the renal function until the patient’s urinary system was totally exenterated.

Abstract
INTRODUCTION:
We present an unusual complication following robotic assisted radical hysterectomy.
CASE REPORT:
A 51-year-old female with stage 1B1 cervical cancer underwent a robotic assisted radical hysterectomy. The procedure was prolonged with difficulties dissecting the left parametrium and vaginal fornix with persistent bleeding from the left vaginal vault. Post-operatively the patient was electively sedated and ventilated. Extubation was difficult due to patient agitation but achieved on day 2. Agitation persisted and a head CT scan was performed and a diagnosis of cerebral oedema was made.
DISCUSSION:
Factors contributing to this case include prolonged operating time, prolonged Trendelenburg position with high pressures of CO2 pneumoperitoneum and excessive blood loss. These factors may contribute to poor cerebral venous outflow, increasing intracranial pressure leading to increased risk of cerebral oedema.
CONCLUSION:
The mechanics of robotic assistance may be used to reduce these risks by significantly reducing intra-abdominal pressure improving venous return. The use of robotics in surgery has been increasing over the last 10 years, and the benefits have been well documented. We present an unusual complication following robotic assisted radical hysterectomy performed for cervical cancer.

Background: We describe a detailed novel step-by-step approach for creation of an ileal neobladder and compare the outcomes with standart neobladder., 36 consecutive patients with bladder cancer underwent radical cystectomy and orthotopic urinary diversion with an ileal neobladder. A novel technique of ileal neobladder construction, called the Anatolian neobladder, was designed by a single surgeon (ZT). Demographics and clinical data were collected. Perioperative, oncologic, and functional outcomes were reported. Complications were graded as early or late. These outcomes were compared with patients who underwent standard neobladder during this period in our center.
