LIIveWell Wellness Program

Lennox and WebMD have partnered to offer you LIIveWell, an online wellness program that is FREE for ALL Lennox employees and their families.

WebMD 1-on-1 Health Coaching

It’s easy to get started with your FREE WebMD coaching sessions, and your health screening results are a great place to start. With your coach, you will identify opportunities and barriers for your health and set the right goals. Finding a workout buddy, adding more protein to your diet, or simply setting aside 10 minutes of quiet time in the afternoon are just a few goals examples – it’s all about what you want to improve!

WebMD coaches are certified and specialized in many health and wellness areas, including nutrition, exercise physiology, mental health, and diabetes management.

LIIveWell Annual Health Screenings

Stay healthy with LIIveWell’s FREE annual health screening benefit and avoid the $75 Wellness surcharge(s).

NOTE: If you are not enrolled in a Lennox medical plan, an annual health screening is not required.
Lennox provides an annual health screening benefit to help you gain more insight into your health and avoid paying the $75 Wellness surcharge(s) in the next plan year.*

To qualify for the surcharge waiver, your annual screening results (and those of your covered spouse) must indicate that your physical health has improved or remained within the healthy target range year over year.

What is a Health Screening?

A health screening is a 15 – 20 minute confidential assessment of your physical health, lifestyle, and well-being. This annual check-up will help you identify potential health risks early, take action before symptoms appear, and make informed decisions about your health.

* There is a separate additional $75 per month Wellness surcharge for your covered spouse.

Lennox medical plan members who enrolled before August 1 are eligible for the free annual health screening.

To avoid the Wellness surcharge(s) in 2025…
  • You, and your covered spouse, must each complete and submit your annual health screening results in LIIveWell to confirm 3 of 5 measured health metrics have improved or have remained within the healthy target range by September 30, 2024.*
  • New hires, and their covered spouses, are each required to complete an annual health screening during the first year of employment to set a baseline. Baseline health metrics are not required to be in the healthy target range. Submit your screening results in LIIveWell by September 30, 2024.*
* If you and/or your spouse are unable to complete the requirements due to a medical condition, your physician can fill and sign the Medical Waiver form. The completed form(s) can be submitted via email, fax, or directly uploaded into your LIIveWell account by November 29, 2024.

* If you or your spouse are pregnant, you do NOT need to complete a health screening. Instead, you must complete and submit the Pregnancy Waiver form via email, fax, or directly uploaded into your LIIveWell account by November 29, 2024.

LIIveWell’s annual screening measures the 5 key health metrics below against your applicable target goals.

Both you and your covered spouse must meet or improve 3 out of 5 health screening metrics to avoid the $75 per person per month Wellness surcharge in the following plan year.

LIIveWell offers three convenient and confidential ways to complete your health screenings. You can also inquire about alternative options by calling (844) 800-2454.
  • Onsite Health Screening
    Lennox coordinates onsite health screening events with licensed medical providers. Check your LIIveWell account or contact your local HR representative for dates/times. The screening appointment takes 15 – 20 minutes, and your results are automatically submitted.
  • LabCorp Facility
    Visit your LIIveWell account to locate a nearby LabCorp that offers “Employee Wellness with Body Measurements.” Download the Lab Requisition Form and present it at the lab. LabCorp will automatically submit your screening results.
  • Visit Your Doctor
    Print the Physician Form on LIIveWell and bring it with you to your in-person doctor’s appointment. Ask your doctor to complete the form during your exam. You can upload your signed Physician Form in your LIIveWell account (preferred method). Alternatively, you or your doctor can submit the form by fax or mail.
It can take up to 10 days for your health screening results to be visible in your LIIveWell account. If you still do not see your results after 10 days contact LIIveWell at (844) 800-2454.

NOTE: First time LIIveWell program participants will need to register.

Track your progress towards meeting your annual health screening requirements.

LIIveWell will update your status when you (and/or your spouse) achieve 0%, 50%, or 100% task completion.

  • 0% – Health screening not completed.
  • 50% – Health screening completed but did not pass because your results are not within the target range.
  • 100% – Health Screening completed, you achieved meeting 3 of 5 health metrics.

Find and click the “Completed” tab to view information about your completed health screening tasks and results for each measured metric. You can also see and compare your previous screening results.
