If your dependent was covered under any Lennox benefit plans

• Contact BenefitSource within 31 days from the death.


• Submit a copy of the death certificate to BenefitSource within 60 days.


• Check your Life Insurance policies and listed beneficiary information. Call Voya Financial for help.


• Check your listed beneficiary information on your 401(k) Retirement Plan. Call Fidelity for help.


• If participating in the FSAs, consider adjusting the pre-tax amount(s) you contribute.
