Loss or Gain of Coverage

Qualifying events include losing outside benefits or becoming eligible for another employer’s health plan.

Within 31 days, you or your dependents can enroll in or change Lennox’s plans based on the following reasons:
  • Losing Your Source of Coverage
    If you lose health care coverage under a spouse’s plan or another outside plan, you may qualify for a special mid-year enrollment period to elect Lennox benefits.
  • Losing Program Eligibility
    Public programs like Medicaid and CHIP provide health care insurance to individuals and families that cannot afford it themselves. If you currently rely on one of these programs but you lose eligibility, you may qualify for a special mid-year enrollment period to elect Lennox coverage.
  • Gaining Health Care Coverage
    If you or a covered dependent become eligible for other health care coverage through a different employer or become entitled to Medicare/Medicaid during the plan year, you may be able to drop Lennox coverage.

Evidence of Coverage Loss or Gain

Proof of the qualifying life event must also be provided on official letterhead within 60 days from the employer, Medicaid/CHIP, public health care exchange, Medicare, etc. stating the following:
  • Name of employee (and dependents if applicable)
  • Name of insurer/employer from where coverage was gained/lost
  • Type of coverage gained/lost
  • The date coverage was gained/lost matching the life event date
NOTE: If adding a new dependent, their benefits will not be effective until documentation is received and approved.

Your Next Steps

  • Within 31 days after the qualifying life event, review your current benefit elections and request changes in BenefitSource. You can add or remove your spouse and/or eligible dependents from Lennox coverage.
  • Submit required documentation in BenefitSource within 60 days of making benefit changes (e.g. marriage license, dependent verification, divorce papers). New coverage will be begin once approved.
  • Plan and budget for any new applicable costs that will be deducted from your paycheck.
  • Contact Quantum Health for 1-on-1 assistance if enrolling in the medical plan for the first time.
  • Consider enrolling in the FSAs or adjusting your current pre-tax contribution amount(s).
