    What if your next meal depended on skipping a doctor’s visit? This is the reality for millions in urban poor communities across India. Forget vitamins and exercise—'wellness' for them means simply making it to tomorrow. This harsh reality is a key reason why most health initiatives fail in India’s poorest communities. Missing work for a doctor's visit or taking time to recover is often unthinkable. When illness strikes, quick recovery becomes essential, sometimes leading to the acceptance of untested treatments. These communities are particularly vulnerable to communicable diseases, and their daily challenges make adhering to ongoing treatments difficult, especially once immediate symptoms subside. The daily dilemma of ‘Survival vs. Wellness’ in these communities compelled us to rethink health strategies and program design. Our trademarked "Currency of Wellness" framework is helping organizations understand what being "well" truly means for these communities, beyond just medical definitions. Here are two examples of how this framework has driven positive outcomes: 1. Pulmonary #TB #Treatment: Defining contextual, personal recovery markers that patients can identify with during the Continuous Phase of treatment improves adherence and completion. 2. #NCD #Management: For conditions like hypertension and diabetes, the Currency of Wellness has helped pinpoint focus areas for long-term behavior change. Looking ahead, we’re using this approach to: 1. Deeply understand contextual recovery trajectories and train AI to improve patient communication. 2. Identify various non-medical factors that aid recovery. 3. Develop intersectional collaborations to design services that truly meet community needs.     TB care and delivery needs to get closer to patient and communities for us to successfully eliminate it. The tongue swab will be a potential game change in helping identify a lot of vulnerable TB cases much sooner, bringing diagnostic tech closer to the patient, especially in low-resource settings. Treemouse had partnered with Clinton Health Access Initiative, Inc. on assessing the adoption feasability of a tongue swab as well as 2 other low-tech, diagnostic tools that are under-development. Our study had covered India and South Africa, accounting for almost half of the world's TB burden. We assessed how readily would the healthcare system (public and private) adopt these tools, where would they fit in with respect to the current protocols and what kind of regulatory support was needed to introduce them. Sputum collection remains an unpleasant experience for the patient and the health workers involved in the process. It is therefore very encouraging to see the technology maturing to deliver on patient-centric TB care. #patientcentrictbcare #tbmuktbharat #humancenteredstrategy #healthcare

    Tounge swabs is a promising additional sample type with potential to enable TB testing at the primary health care level or in communites. In this new paper we demonstrate that Mtb from the tongue dorsum biofilm are viable and can be grown in culture, an important piece in our understanding of what this new sample actually captures.     Have you interacted with a chatbot on WhatsApp before? How has your experience been? Is it easy to navigate? Is the learning journey of discovering a new feature too complex? After one use, do you feel the need to return to the chatbot every time you have a query? These questions may seem odd when considering that a bot is meant to be a simple mechanism that enhances our experience of receiving information. But beneath the surface lies a vast machinery and the minds of ‘designers and thinkers’ who make every move seem intuitive, as if it has been 'designed for you'. Now, imagine designing digital products for Bharat, a nation teeming with diversity – diverse cultures, languages, contexts, incomes, and educational backgrounds. How do you design digital products for the public at large that are functional, usable, and accessible, catering to all these differences in a country like India? What it requires is the ability to see the invisible - to understand the deeply hidden values, motivations, and needs of users. What it needs is an understanding of the science of human behaviour. These are the types of challenges that we love to solve at Treemouse. We are partnering with Samagra | Transforming Governance to host a track on design for Digital Public Goods (DPG) for the next DPG Dialogues. We will be working closely with the organisations that are part of the Code for GovTech (C4GT) initiative and will be mentoring them on their usability-led challenges.     I wrote a short piece for ARMMAN's blog sharing some anecdotes and reflections from our user research in Jharkhand. I think the last few lines come very close to capturing why I do what I do. "When we asked Sadhana what motivates her to continue to learn, she said it was important for her to keep getting better at her job. ‘When I do my job well, I help my gaanv-samaaj (village and society) move forward.’ It is ASHAs like Sadhana, and countless pregnant women like Tabassum and Swati, who motivate us to keep improving our mHealth programmes like Kilkari and Mobile Academy. In a world where most emerging technologies are built largely by men, for men, and even tested on men, we want to make sure that the voice of our users shapes the design and development of our products at every step. From making our gender-transformative content available in multiple languages to ensuring that an illiterate user can navigate our products using visual cues alone, our iterative, human-centred and evidence-based innovation process puts women at the centre. Like Sadhana, we too believe that when women do well, society moves forward." (These are snippets from user research studies done in collaboration with PeepalDesign and Treemouse)

    Building innovative healthcare solutions to meet the needs of our communities requires collaboration and a deep understanding of how we can unite to develop effective approaches for the people. In a recent knowledge exchange session, Treemouse and Noora Health, two organizations deeply committed to understanding patients' attitudes, came together to explore the intricacies of engaging with communities in the healthcare system. Treemouse shed light on their patient-centered care approaches. We shared how we've influenced the guidelines on patient needs and treatment support across key stages of TB treatment, especially for Adverse Drug Reactions. Noora Health provided an overview of their current programs, emphasizing their perspective on maintaining engagement with patients' families in their homes. They underscored the importance of leveraging digital channels for routine interaction across their campaigns. The discussion centered on the significance both organizations place on gaining a holistic understanding of a patient’s ecosystem. Noora Health's Care Companion Program and our focus on building contextual solutions for the community prompted an exchange of thoughts on engaging patients through their family members.
