    🔬 The Importance of Diagnosing Macrocytic Anemias and the Role of AI in Differentiation Continuing our series on intriguing medical cases diagnosed using SigTuple's AI100, today we explore the significance of identifying and diagnosing macrocytic anemias and the vital role of AI in differentiating between round and oval macrocytes. Understanding Macrocytosis: Macrocytosis refers to a condition where red blood cells (RBCs) are larger than normal, typically indicated by an elevated Mean Corpuscular Volume (MCV). While MCV values ranging from 80 to 100 femtoliters (fl) are considered normal, larger RBCs can indicate macrocytosis, a condition that may or may not be associated with anemia. Types of Macrocytic Anemia: Megaloblastic Anemia: Characterized by the presence of oval macrocytes, commonly caused by vitamin B12 or folic acid deficiency. Non-Megaloblastic Anemia: Identified by round macrocytes, with causes including alcoholism, liver disease, hypothyroidism, and more. Case Study: In a recent case, a patient with macrocytic anemia presented with round macrocytes. The AI100, powered by Shonit, accurately flagged and classified these round macrocytes, showcasing its advanced capabilities. This differentiation is crucial for the pathologist and clinician to manage the condition effectively. Laboratory Findings: Hemoglobin (Hb): 9.7 g/dl MCV: 102.7 fL The AI100 provides visual evidence and ICHS grading, enhancing the accuracy and efficiency of diagnosing macrocytic anemia. The Role of AI in Diagnosis: Identifying the cause of macrocytosis can be challenging, requiring a systematic approach that includes clinical history, physical examination, complete blood count (CBC), and peripheral smear examination. The AI100 supports pathologists by providing precise, visual-based evidence, ensuring accurate differentiation and interpretation of macrocytic anemia.     What an exhilarating event it was! Going into ADLM, one of the largest global trade shows for laboratory medical devices, we were nervous indeed. AS76 has been years of hard work and sweat from the entire team, and this was the first time we were showcasing our product to the industry. Over the last week, we had a chance to engage with eminent pathologists, dominant medical device distributors and giant global OEM's, and to hear from these industry titans that "AS76 would now be the new benchmark for microscopic review of peripheral blood smears " was the most satisfying thing for us to hear. For the last 12 months, we have been our biggest critic - from robotics to optics to data science to engineering and product - every team has pushed themselves to the limit to ensure that we can redefine microscopy for the times to come - and it really showed in the feedback we received.     🔬 An Interesting Case of AML with Monocytic Differentiation We are excited to kick off a series where we share intriguing medical cases diagnosed using the SigTuple AI100. Our first case highlights the powerful diagnostic capabilities of our technology. Case Overview: A 57-year-old male presented with a two-month history of fever. A peripheral smear was analyzed using the SigTuple AI100, revealing the presence of blasts along with 21% cells with monocytic differentiation composed of promonocytes, monoblasts, and monocytes. CBC Findings: • Hemoglobin (Hb): 9.4 g/dl • WBC count: 38.5 x 10^3 /uL • Platelet count: 127 x 10^3 /uL Flow Cytometry Results: 60% of the blasts gated were positive for CD13, CD33, cMPO, HLA-DR, CD11b, CD64, CD123 AND CD38. NEGATIVE FOR T-CELL MARKERS, cCD3, cCD79a, CD34, CD20, KAPPA/LAMBDA, CD56, AND CD16. Impression: Acute Myeloid Leukemia (AML) with monocytic differentiation. Discussion: Acute myelomonocytic leukemia (AMML) is a distinct subtype of AML characterized by the proliferation of both neutrophil and monocyte precursors. According to the 2022 WHO classification, AMML accounts for 5-10% of AML cases and is more common in older individuals, with a median age of 50 years and a male-to-female ratio of 1.4:1. Key Features of AMML: • Evidence of both granulocytic and monocytic differentiation • Blasts/blast equivalents (myeloblasts, monoblasts, promonocytes) must comprise at least 20% of blood or bone marrow cells • Granulocytic differentiation is present but cells of monocytic lineage must be at least 20% of cells Microscopic(histological) Description: • Myeloblasts: High N:C ratios, round nuclei, variably granular cytoplasm, occasional Auer rods • Monoblasts: Large cells with abundant, moderately to intensely basophilic cytoplasm. May have pseudopod formation, scattered fine azurophilic granules, and vacuoles. Often with round nuclei with lacy chromatin and one or more large nucleoli • Promonocytes: have more irregular nuclei, typically with delicate folds and abundant, less basophilic cytoplasm which is more obviously granulated with occasional large azurophilic granules and vacuoles. This case underscores the critical role of advanced diagnostic tools like the SigTuple AI100 in identifying complex medical conditions accurately and efficiently.
