Thank you to our partners at the City of Toronto, Fred Victor, and United Way Greater Toronto for your contributions in making this bold and innovative project possible. Together, we have created an initiative that will help tenants reach better health and well-being outcomes, reduce Emergency Department visits, and hospital readmissions by providing permanent housing and on-site supports that are tailored to the needs of each tenant.

Dunn House is a four-storey modular building with 15 accessible homes and 36 standard homes. The building includes a variety of amenities such as shared laundry, a commercial kitchen, a dining area, a communal area, a programming space, and a clinical room. There are also on-site social and health services within the building, to contribute to tenants' well-being and housing stability.

🎉 Today marks the opening of Dunn House, Canada’s first hospital-led supportive housing initiative. In addition to housing, this building will provide on-site social and health services for 51 of UHN’s most medically and socially complex patients, who are unhoused. Dunn House tenants were selected by cross-referencing with the City’s list of all individuals experiencing homelessness in Toronto. The selection process for tenants was then further assessed and prioritized based on utilization of emergency health services, individual needs, circumstances, and other criteria. The historic opening of Dunn House would not be possible without the partnership of City of Toronto, Fred Victor, and United Way Greater Toronto, and support from all levels of government.

Thank you Chief Demkiw for sharing your incredible story and appreciation for your care team at Princess Margaret Cancer Centre. We are honoured to have been part of your cancer care journey 💙
I don’t often share personal posts here, but today I am filled with gratitude after my wife and I visited Princess Margaret Cancer Centre to learn that after 5 years I am cancer free. To my family members & colleagues, and my care team: thank you for everything. Read my story: https://lnkd.in/gHgRwDfn

Today, on International Day of Older Persons, we want to shine a spotlight on our incredible NORC community leaders like Sue and Nadine! 🌟 These passionate older adults lead the way in building supportive, connected communities. Through their leadership, dedication, and lived experience, they help shape programs that truly meet the needs of their neighbours and create spaces where everyone can thrive as they age. We are so grateful for the energy and heart they bring to our UHN NORC Innovation Centre initiatives—together, we're making aging in place a reality. #InternationalDayOfOlderPersons ➡️Learn more: https://uhn.ca/norc
