The NDIA and our Local Area Coordinators check in with participants around 2 months before the reassessment date outlined in your plan. We will contact you to organise this check in, and let you know how you can prepare. If things are going well and no changes are needed, we may renew your existing plan for a longer duration. If your circumstances have changed we will work with you to review your NDIS plan at a follow up meeting. After your check in, we’ll send you a summary of what we talked about and start any next steps we’ve agreed upon.
