Salivary Metabolites in Breast Cancer and Fibroadenomas: Focus on Menopausal Status and BMI

This study of the features of the biochemical composition of biological fluids in patients with breast cancer, including saliva, allows us to identify some indicators as metabolic predictors of the presence of the disease. Objectives: to study the influence of the menopause factor and body mass index (BMI) on the biochemical composition of saliva and to evaluate the applicability of metabolic markers of saliva for the diagnosis of breast cancer. Methods: The case–control study involved 1438 people (breast cancer, n = 543; fibroadenomas, n = 597; control, n = 298). A comprehensive study of the biochemical composition of saliva was carried out using 36 parameters. Results: When comparing the salivary biochemical composition in breast cancer, fibroadenomas, and controls, it is necessary to take into account the menopausal status, as well as BMI (less than 25 or more) for the group of patients with preserved menstrual function. A complex of biochemical parameters has been identified that change in saliva during breast cancer, regardless of menopause and BMI (total protein, urea, uric acid, NO, α-amino acids, GGT), as well as specific parameters that must be taken into account when analyzing individual subgroups (imidazole compounds, LDH, catalase, α-amylase). During the study of a separate group of patients with leaf-shaped (phyllodes) tumors, we found similarities with breast cancer in the changes in some biochemical parameters that can be attributed to metabolites of malignant growth (protein, α-amino acids, calcium, NO, pyruvate, peroxidase, α-amylase). Conclusions: We demonstrated changes in a wide range of salivary biochemical parameters depending on the presence of fibroadenomas and breast cancer. From the point of view of clinical practice, this may be useful information for monitoring the condition of patients with fibroadenomas, which are difficult to unambiguously classify based on instrumental diagnostics alone. Modern mediation analysis techniques supplement the primary intention-to-treat analysis with the aim to shed light onto the treatment mechanism. We investigate to what extent the anatomic marker vitreomacular adhesion resolution (VMAR) mediates vision benefits, comparing ocriplasmin vs. a sham regimen. A causal mediation analysis is applied to randomized trial data including 218 participants with vitreomacular traction. Logistic regression models are used to estimate the total treatment effect (TTE) on binary outcomes. Outcomes, assessed at month 24, included visual acuity improvement (VA-I): ≥2-line increase in VA; visual function questionnaire improvement (VFQ-I): ≥5-point increase in the 25-item visual function questionnaire composite score (VFQ-25cs); visual function improvement (VF-I): defined as either a VA-I or a clinically meaningful improvement in the VFQ-25cs. Quantity of interest is the breakdown of TTE into an indirect (through VMAR) and direct effect to estimate the extent to which the TTE is transmitted through the mediating variable (VMAR) vs. other pathways. Causal effects are expressed as risk differences. Indirect effects for VFQ-I, VA-I, and VF-I are 5.7%, 11.8%, and 5.2%, respectively, representing the increase in the probability of a vision improvement if VMAR status were changed for each participant to the extent that it is affected by ocriplasmin. The direct effects are 8.3%, 12.1%, and 24.1% respectively, capturing the effect of treatment on the probability of a vision improvement if ocriplasmin left each participant’s VMAR status unchanged. The relative treatment effect of ocriplasmin on the functional outcome VA-I is to a large extent the result of its effect on VMAR, while an improvement in the patient-reported outcomes VFQ-I or VF-I was only partially mediated by VMAR. The purpose of this study is to explore the key roles and functions of institutions or organizations in the process of the spatial activation and industrial transformation of leisure fishing villages and to summarize this operating model. This study takes Xunpu Village, Quanzhou City, Fujian Province as the research object, which is well-known for its recreational fisheries. It mainly uses case study methods and uses on-site surveys and interviews to understand the formation time, spatial distribution, and business type information of the Zanhuawei shops in Xunpu Village. Zanhuawei, which refers to the traditional hairstyle of Xunpu women, was later applied for national intangible cultural heritage status in China. After the outbreak, the spread of Zanhuawei on the Internet inspired a large number of tourists to visit Xunpu Village to experience Zanhuawei. The study found that (1) Zanhuawei industry replaced traditional fishery and became the leading industry type in Xunpu Village. (2) The operating model led by the government and integrating public and private funds has had a stimulating effect on the revitalization of local commercial and residential space, and a large number of idle spaces have been reactivated. (3) The cooperation model between a large number of investors and local fisherwomen has completely changed the economic model of the village. Zanhuawei industry mobilized all official and media resources to promote spatial activation at various scales, including regions, settlements and commercial spaces. Full article

Background/Objectives: Fibromyalgia (FM) affects up to 5% of the global population and is a leading cause of significant social and economic consequences. Higher health literacy leads to better understanding of treatment plans, improved self-care, and adherence to recommendations, enhancing overall quality of life. This study aims to determine whether different aspects of the disease are influenced by patients’ education level and literacy when applying the same therapy and to assess how patients’ perceptions of therapy outcomes vary over time based on their educational level. Methods: This study involved 140 fibromyalgia (FM) patients diagnosed using the 2016 ACR criteria, with 128 completing the study. Participants attended three visits over 28 weeks and were stratified into four groups based on educational level: Group 1—secondary school or less; Group 2—high school graduates; Group 3—college graduates; Group 4—university graduates. Patients were assigned to groups (n = 32, 32, 30, and 34, respectively) after the initial evaluation (T0). The treatment was assessed (T1) and followed up three months later (T2) to evaluate changes in functional status and quality of life. All patients underwent the same rehabilitation program, cognitive therapy, and kinesiotherapy. Results: Significant differences in disease impact on the patient’s life (FIQ total score) were observed between groups from the initial evaluation (p = 0.000). The overall FIQ score was notably affected by non-pharmacological therapy in patients with higher education. These differences continued to be significant even three months after the treatment ended (p = 0.000). Functional limitations were evident from the start (p = 0.000) and improved significantly post-treatment in patients with higher education (p = 0.000). However, subjective evaluations of disease impact (assessed by the first item of FIQ) did not consistently align with objective findings (hand grip strength). Functional limitations did not significantly differ in subjective evaluations (F1Q1) across educational levels (p = 0.045), and inverse correlations were noted between functional status and SF-12 well-being components. Conclusions: This study underscores that higher education enhances fibromyalgia management and functional outcomes, particularly when combined with non-pharmacological therapies. However, subjective perceptions may not always align with objective improvements, indicating that factors beyond education, such as personal and external influences, also impact disease management. Thus, improving health literacy through educational interventions could further benefit FM patients’ quality of life. Full article

Weituo 韦驮 is one of the most frequently represented protective deities in Chinese Buddhist temples. The statue of Weituo is enshrined virtually in every standard Buddhist temple in China today, and at a unique location—in the entrance hall for the four Heavenly Kings and facing inward. Why is the Weituo statue placed at this place and looking inward? The Chinese Weituo can be traced back to the Hindu god Skanda. However, his iconography and functions in China were completely reshaped by master Daoxuan’s 道宣 (596–667) account of this deity. Actually, Weituo is just one of the many Buddhist protective deities for general protection and prosperity. In China, as I demonstrate in this study, Weituo has been known as the most diligent Dharma-protector and is associated in particular with the saṅgha, as a patron guardian and inspector of monks and nuns. Thus, Weituo’s images are shrined in Buddhist temples. The locations of these images reflect the importance of Weituo and his special role in Chinese Buddhism. In the past, there have been a few studies, but they mainly focus on his origin. The functions of Weituo and the worship of Weituo in the Chinese saṅgha have not been well examined. Going beyond his identities, iconography, and style, this study aims to contextualize Weituo images in temple space and religious practice. Full article

Prognostic Impact of Statins in Heart Failure with Preserved Ejection Fraction

Background: Heart failure (HF) with preserved ejection fraction (pEF) has lacked effective treatments for reducing mortality. However, previous studies have found an association between statin use and decreased mortality in patients with HFpEF. The aim of this study was to analyse whether statin therapy is associated with a reduction in mortality in these patients and whether the effect differs according to the presence or absence of ischaemic heart disease (IHD). Methods: We analysed data from the National Registry of Heart Failure, a prospective study that included patients admitted for HF in Internal Medicine units nationwide. Patients with HFpEF were classified according to the use of statins, and the differences between the two groups were analysed. A multivariable analysis was performed using Cox regression to assess factors independently related to mortality. Results: A total of 2788 patients with HFpEF were included; 63% of them were women with a mean age of 80.1 (±7.8) years. The statin-treated group (40.2%) was younger, with better functional status, and had a more common diagnosis of vascular disease and lower frequency of atrial fibrillation. The most frequent aetiology of HF in both groups was the hypertensive one. Nevertheless, ischaemic HF was more common in those who received statins (24.8% vs. 9.6%; p < 0.001). Multivariable analysis showed lower mortality at the 1-year follow-up in statin-treated patients (OR: 0.74; 95%CI: 0.61–0.89; p = 0.002). This association was observed in patients without IHD (p < 0.001) but not in those with IHD (p = 0.11). Conclusions: Statins are associated with a decrease in total mortality in patients with HFpEF. This benefit occurs mainly in those without IHD. Full article

This study explores the recent emergence of traditional Korean houses and hanoks as markers of cultural capital in Seoul, South Korea. While the ownership of detached houses itself can be a symbol of wealth in Seoul, traditional-style houses have become increasingly associated with luxurious living, particularly after the successful remodeling of hanoks in metropolitan settings such as Bukchon in Seoul. This study employs the critical luxury studies method to analyze the recent rise in hanok construction/remodeling among elites, and illustrates how traditional architectural forms have become status markers. Although the regeneration of traditional houses in cities has been examined from the perspective of gentrification or touristic cultural consumption, less academic attention has been placed on the phenomenon from the perspective of the homeowners’ taste. This study examines how traditional architecture has become a form of acceptable luxury through a media analysis of published articles and interviews with the residents of hanoks. This study argues that protecting endangered traditions and rich sensory experiences function as important moralizing factors in luxury housing, indicating that sociocultural valuation becomes as significant as market valuation. Background/Objective: Sensitization to specific IgE Staphylococcus aureus enterotoxins (SEs) is associated with an increased risk for severe asthma development. Limited data exist regarding the association of seropositivity for specific IgE SEs and the different aspects of severe asthma. We aimed to determine whether the presence of SEs is associated with asthma-related parameters such as inflammatory cells in the airways, features of airway remodeling, and other variables relating to asthma assessment and severity. Methods: Fifty patients with severe asthma were recruited in the study. Demographics, comorbidities, asthma duration, and asthma medication were recorded by treating physicians. Specific IgE SE measurement, lung function, atopic status, asthma control test (ACT), sputum induction, bronchoscopy with BAL, and indices of airway remodeling were also assessed. Results: Twelve patients were positive to enterotoxin sensitization. Patients seropositive to specific IgE SEs significantly differed in regard to FEV1% pred and FEV1/FVC ratio compared to seronegative ones. Analyzing the inflammatory variables obtained from induced sputum, BAL, and endobronchial biopsies, the only significant difference was that of smooth muscle area (SMA), which was greater in specific IgE SE seropositive patients. The multivariate linear regression analysis showed two significant associations of specific IgE SE seropositivity. We found a negative with FEV1% pred with beta standardized coefficient 95%CI −0.054 (−0.083, −0.031), p < 0.001, and a positive with SMA with beta standardized coefficient 95%CI 0.054 (0.081, 0.037), p < 0.001. Conclusions: Seropositivity to specific IgE SEs in severe asthma is associated with more severe airflow limitation, obstruction, and upregulation in SMA, indicating a possible role in the remodeling process. Pumped storage units serve as a crucial support for power systems to adapt to large-scale and high-proportion renewable energy sources by providing a stable and flexible energy supply. However, due to the coupling effects of electric power load demands and the complex multi-source factors within the water–mechanical–electrical system, the interrelationship between unit parameters becomes more intricate, posing significant threats to the operational reliability and health status of the units. The complexity of fault diagnosis is further aggravated by the intricate and varied nature of fault characteristics, as well as the challenges in signal extraction under conditions of strong electromagnetic interference and high noise levels. To address these issues, this paper proposes a novel data-model hybrid-driven strategy that analyzes vibration signals to achieve rapid and accurate fault diagnosis of the units. Firstly, the spectral kurtosis theory is employed to enhance the traditional empirical mode decomposition, achieving optimal decomposition and noise reduction effects for vibration signals. Secondly, the intrinsic mode functions (IMFs) obtained from the decomposition are reconstructed, and the entropy values of effective IMFs are calculated as fault feature vectors. Subsequently, the CNN-LSTM model is utilized for fault diagnosis. The effectiveness and feasibility of the proposed method are verified through actual operational data from pumped storage units in a specific region. Through analysis, the fault diagnosis accuracy of the method proposed in this paper can be maintained above 95%, demonstrating robustness in complex engineering environments and effectively ensuring the safe and stable operation of pumped storage units. UAV Anomaly Detection Method Based on Convolutional Autoencoder and Support Vector Data Description with 0/1 Soft-Margin Loss

Unmanned aerial vehicles (UAVs) are becoming more widely used in various industries, raising growing concerns about their safety and reliability. The flight data of UAVs can directly reflect their flight health status; however, the rarity of abnormal flight data and the spatiotemporal characteristics of these data represent a significant challenge for constructing accurate and reliable anomaly detectors. To address this, this study proposes an anomaly detection framework that fully considers the temporal correlations and distribution characteristics of flight data. This framework first combines a one-dimensional convolutional neural network (1DCNN) with an autoencoder (AE) to establish a feature extraction model. This model leverages the feature extraction capabilities of the 1DCNN and the reconstruction capabilities of the AE to thoroughly extract the spatiotemporal features from UAV flight data. Then, to address the challenge of adaptive anomaly detection thresholds, this research proposes a nonlinear model of support vector data description (SVDD) utilizing a 0/1 soft-margin loss, referred to as L0/1-SVDD. This model replaces the traditional hinge loss function in SVDD with a 0/1 loss function, with the goal of enhancing the accuracy and robustness of anomaly detection. Since the 0/1 loss function is a bounded, non-convex, and non-continuous function, this paper proposes the Bregman ADMM algorithm to solve the L0/1-SVDD. Finally, the difference between the reconstructed and the actual value is employed to train the L0/1-SVDD, resulting in a hypersphere classifier that is capable of detecting UAV anomaly data. The experimental results using real flight data show that, compared with methods such as AE, LSTM, and LSTM-AE, the proposed method exhibits superior performance across five evaluation metrics. Background/Objectives: Fibromyalgia (FM) is a syndrome characterized by widespread chronic pain as the primary symptom. Neurophysiological pain mechanisms, such as the function of the descending inhibitory system, are impaired in this condition. The main objective of this study was to compare the results of two paradigms to evaluate CPM in women with FM. The secondary objective was to correlate the results of each CPM paradigm with the clinical status of patients with FM. Methods: One hundred and three FM women were divided into two groups: fifty patients diagnosed with FM were assigned to the conditioned pain modulation (CPM) group using a cold pressor stimulus, and fifty-three patients were assigned to the CPM group using the ischemic pressure stimulus. The main outcome measures were pain intensity, disability, mechanical hyperalgesia, and CPM. Results: The primary analysis revealed significant differences between the results obtained from the different CPM protocols. Poorer outcomes in the cold pressor test correlated with higher pain intensity and a greater disability index. Conclusions: Pain modulation abnormalities in FM patients were evident when using either the cold pressor or ischemic pressure stimuli to establish the CPM paradigm. The cold pressor conditioning stimulus elicited a stronger response than the ischemic pressure stimulus in FM patients. Full article

Shelter forest systems in the sandy areas mainly comprise farmland shelter forests (FSF) and windbreak and sand-fixing forests (WSF). Through a questionnaire survey of farmers in the oasis–desert transition zone of the windy desert areas of the Hexi Corridor in China, a perception assessment model of farmers’ perception of the status quo and ecosystem service function of shelter forests was constructed, and the willingness of farmers to pay for shelter forest construction and protection was measured. Influencing factors for the farmers’ perception of shelter forests were analyzed by a multiple regression analysis. The results showed problems with pests and diseases all present in FSF and WSF, the destruction of farmland in FSF, and water scarcity in WSF. Farmers evaluated the ecological role of shelter forests as the most obvious in windbreak and sand fixation; intermediate in the four ecological roles of leisure and recreation, agricultural production, a sense of locality, and climate regulation; and the smallest in soil improvement. Nearly 95% of farmers are willing to donate funds to shelter forest construction and protection, and the average amount the farmers were willing to pay was CNY 54.30 per year. Willingness to pay, annual household income, gender, environmental experience, age, and interview area have significant effects on farmers’ perception of shelter forests. Finally, in this paper, we recommend increasing motivation through government financial support and training for farmers to address pests and diseases, water security, and the destruction of farmland in shelter forests to ensure healthy and sustainable growth. Background and Objectives: This scoping review evaluates the use of wearable sensor technologies for workplace safety and health monitoring in industrial settings. The aim is to synthesize evidence on the impact of these sensors and their application in high-risk environments. Materials and Methods: Following the PRISMA guidelines, a systematic search across four international electronic databases yielded 59 studies, of which 17 were included in the final review. The selection criteria involved studies that specifically utilized wearable sensors to monitor various health and environmental parameters relevant to industrial workers. Results: The analysis categorizes wearable technologies into five distinct groups based on their function: gas monitoring technologies, heart rate and physiological data collection, fatigue and activity monitoring, comprehensive environmental and physiological monitoring, and advanced sensing and data collection systems. These devices demonstrated substantial benefits in terms of early detection of health risks and enhancement of safety protocols. Conclusions: The review concludes that wearable sensor technologies significantly contribute to workplace safety by providing real-time, data-driven insights into environmental hazards and workers’ physiological status, thus supporting proactive health management practices in industrial settings. Further research is recommended to address the challenges of data privacy, sensor reliability, and cost-effective integration to maximize their potential in occupational health safety. Full article

An Anti-Invasive Role for Mdmx through the RhoA GTPase under the Control of the NEDD8 Pathway

Mdmx (Mdm4) is established as an oncogene mainly through repression of the p53 tumour suppressor. On the other hand, anti-oncogenic functions for Mdmx have also been proposed, but the underlying regulatory pathways remain unknown. Investigations into the effect of inhibitors for the NEDD8 pathway in p53 activation, human cell morphology, and in cell motility during gastrulation in Xenopus embryos revealed an anti-invasive function of Mdmx. Through stabilisation and activation of the RhoA GTPase, Mdmx is required for the anti-invasive effects of NEDDylation inhibitors. Mechanistically, through its Zn finger domain, Mdmx preferentially interacts with the inactive GDP-form of RhoA. This protects RhoA from degradation and allows for RhoA targeting to the plasma membrane for its subsequent activation. The effect is transient, as prolonged NEDDylation inhibition targets Mdmx for degradation, which subsequently leads to RhoA destabilisation. Surprisingly, Mdmx degradation requires non-NEDDylated (inactive) Culin4A and the Mdm2 E3-ligase. This study reveals that Mdmx can control cell invasion through RhoA stabilisation/activation, which is potentially linked to the reported anti-oncogenic functions of Mdmx. As inhibitors of the NEDD8 pathway are in clinical trials, the status of Mdmx may be a critical determinant for the anti-tumour effects of these inhibitors. Full article

The aim of this article is to identify and illustrate the most used psychological techniques in the field of cystic fibrosis (CF) and to help clinicians choose the most appropriate strategy among various possibilities. The disease and its medical treatments can be difficult to tolerate and can cause anxiety about health status or feelings of hopelessness and stress. The prevalence of depression and anxiety is 2.3 times higher in adults with CF than in community samples. A strong correlation has been identified between elevated psychological distress and unfavorable health outcomes, including, among others, impaired lung function, reduced BMI, an increased incidence of pulmonary exacerbations, and an elevated risk of transplantation. The use of psychological interventions is useful in addressing these common distresses in CF patients. Aware of the necessity of identifying efficacious interventions for all levels of depression and anxiety in CF patients, this study presents an overview of the research on psychological interventions for patients with CF, in order to complement the treatments suggested by the international guidelines on mental health in CF cases. In fact, the aim of this study is to conduct a review and quantitative synthesis of the psychological intervention techniques that are currently available for individuals with CF. Full article
