What makes this different? • It looks 30 years ahead, not just the typical 10, giving us a much better view of long-term health. • Factors like kidney function, blood sugar levels, and even social conditions (income, access to healthcare) are taken into account. This makes the tool more personalized and precise. • Also, in a cool move for equity, race is no longer used as a factor, recognizing it’s a social construct, not a biological one. The American Heart Association predicts heart failure will affect more than 8 million Americans by 2030. During American Heart Month, we are highlighting ways Humana has identified opportunities to improve health outcomes and reduce costs for those living with cardiovascular disease. According to a recent Humana Healthcare Research study published in the Journal of Managed Care & Specialty Pharmacy, heart failure patients who delayed filling new prescriptions by more than 30 days after hospital discharge had a 65% increased risk of rehospitalization within 6 months. Delayed filling was also associated with 49.5% increase in average all-cause medical costs. These findings can support further development of post discharge clinical program to improve timely medication adherence after a heart failure event. Patients with heart failure admitted to hospitals with higher 30-day readmission rates and deaths from all causes have a 24 percent higher risk of suffering one or more adverse events during their stay, according to an AHRQ study in Circulation, Cardiovascular Quality and Outcomes. Patients with heart failure admitted to hospitals that perform poorly on mortality and readmission measures have higher risk of acquiring hospital adverse events, the researchers concluded. A hypertensive crisis is a severe spike in blood pressure, divided into two types: 1. **Hypertensive Urgency**: - **Definition**: BP ≥180/120 mmHg without immediate organ damage. - **Management**: Gradual BP reduction with oral medications, no hospitalization needed. 2. **Hypertensive Emergency**: - **Definition**: BP ≥180/120 mmHg with acute organ damage (e.g., heart, brain, kidneys). - **Management**: Immediate hospital care and rapid BP reduction with IV medications.
