🔵 El Dr. Rafael Portela en la Unidade Local de Saúde Gaia e Espinho (Portugal), operó a un paciente de 61 años que presentaba lumbociatalgia bilateral, empeorada en el lado derecho, siendo refractario al tratamiento conservador.
La RM mostró espondilolistesis grado II de L5-S1 por lisis ístmica bilateral de L5.
Vista la situación, se decidió realizar ALIF con fijación percutánea posterior, mediante caja COBRA® + MOSS VRS® MIS Spinal System de Biedermann Motech, distribuido por Medcomtech.
Los resultados fueron muy positivos, con una mejoría total de los síntomas y restauración del balance sagital. La buena evolución de este caso, refuerza el impacto de las técnicas avanzadas en la cirugía de columna, para poder brindar la mejor calidad de vida a nuestros pacientes.

Os resultados foram muito positivos, com melhoria total das queixas e restauro do balanço sagital. A boa evolução deste caso reforça o impacto das técnicas avançadas na cirurgia da coluna, para proporcionar a melhor qualidade de vida aos nossos pacientes.
🔵 Dr. Rafael Portela at Unidade Local de Saúde Gaia e Espinho (Portugal), operated a 61 year-old male patient, who presented bilateral lumbosciatica, worse on the right side, which was refractory to conservative treatment.
The MRI showed grade II spondylolisthesis of L5-S1 due to bilateral isthmic lysis of L5.
Given the situation, it was decided to perform an ALIF (Anterior Lumbar Interbody Fusion) with posterior percutaneous fixation, using a COBRA® cage + MOSS VRS® MIS Spinal System from Biedermann Motech, distributed by Medcomtech.
The results were very positive, with a total improvement of the symptoms and restoration of sagittal balance. The great evolution of this case reinforces the impact of advanced techniques in spinal surgery, in order to provide the best quality of life to our patients.

48 year old women’s league tennis player with a symptomatic left L45 spondylolisthesis with progressive low back pain and recurrent left leg pain after previous micro decompression two years ago.
I did this case this morning outpatient. This post was to highlight the second step, placement of headless MOSS 100 screws with AP fluoro.
I measure screw length on Preop imaging, in this case 45/40 at 4/5.
The approach I use is midline 2/3 inch, so the trajectory tends to be more vertical. I can laterally deviate if I choose by bigger incision or bilateral skin, both with trans-fascial approach.
This particular exercise was the entire 4 minutes 11 seconds.
This is the whole placement using the awl tap for screw position establishment and ultimate placement. I go to lateral to deliver final depth and before bringing in retractor.

Left sided tlif after headless screw placement. Facetectomy done using iMAS methodology based on navigation off of screw placement.
I do indirect decompression with tlif before I do final direct decompression microsurgically.
