Stick to dedicated servers and sexual content is almost non-existent. Beware of community servers though, as porn sprays are common for people to use/abuse ingame.

When a Demoman dominated a Medic, there's a small possibility where he will say "Dominated! ... And I've been shaggin' yer wife! Hahah!".
There is a weapon in the game for soldier called "the original" which is an oddly shaped rocket launcher which comes out in the middle of the screen instead of the side. This is an optional weapon and is just a reskin of the stock, however the projectile is also in the middle so most players use it. The reason it comes out the middle of the screen is because the weapon references the rocket launcher in Quake.
There is a melee weapon for (almost) all classes named the Conscientious Objector that lets the owner show ANY image on the weapon's surface. Including porn.
Players can obtain an item that can have any image applied, along with the feature to "spray" decals onto walls existing. These images are not moderated at all. Both of these can now be turned off in the Advanced Options menu.
The description of the cosmetic "The track terroriser" reads "For a brief stint in high school, Scout joined the track team in one of his many schemes to pick up girls. He was kicked off the team after three days when everyone realized he was 23-years-old and also not enrolled in the school.", however this is not official lore by Valve, as the item was community contributed.
None in the game itself, but players are free to choose any one image from their computer to "spray" onto the gameplay map like a poster for others to see. These sprays can be hidden from view using the console command "cl_playerspraydisable 0". Some servers also ban players if they put up any bad sprays.
If you play on multiplayer servers people may show things that are sexually explicit or say things that are.

It's moderate, there's lots of blood, dismemberment, etc, but it's very cartoony and not realistic.
The Demoman has a "taunt" that involves him using a sword; if an opponent is within vicinity, he decapitates them.
It has a lot of violence and gore but a large amount of it can be removed in properties
Although all the violence and gore is comical and always played for laughs, it still exists and is a somewhat-accurate representation of real violence.
When a player is hurt or killed, their character will scream in pain. All the recordings are comical, though.
More gory than hl2 or csgo but not that realistic.
Players can be shot, blown up, burnt to death, stabbed, eaten by sharks, eaten by crocodiles, cut to pieces by saws, bleed to death, fall to their deaths and be beaten to death.
Lots of weapons in the game violate the rules of war. These include the flamethrower (classified as incendiary weaponry) , the sticky bomb launcher (classified as anti-personnel landmines) and the jarate (classified as chemical warfare)
Non-graphic cartoon violence is shown.
Blood will spray comically when a player is shot. When a player is slashed or blown up, pieces and body parts will fly across the ground, letting blood rain down on the landscape. Most deaths end in the killed player's character model going limp and becoming a "ragdoll", with perhaps some red splotches depending on how much they were injured prior to dying. Blood splatters on walls and/or the ground when characters are shot.

The Sniper can throw jars full of his own urine at enemies. He may exclaim "piss!" when doing so.
When playing in Russian, when a Sniper gets a revenge kill on a Spy, he can say "Take that, you sugary (f-slur) "
Online players using voice chat or text chat may use whatever language the server permits. Text chat in the game can be hidden with the console command "hud_saytext_time 0", and all voice chat can be muted with the command "voice_enable 0".
The characters in the game (mostly the Scout and Engineer) will sometimes say mild swear words such as "hell" and "damn" as voice responses/commands. The Sniper has two lines of dialogue that contain profanity; "This is gonna be a real piece of piss, you bloody fruit shop owners!" and "Thanks for standin' still, wanker!" One of the Spy's lines is the French swear word "merde".
In the Halloween 2013 update, two characters from the comics were added to the game; Redmond and Blutarch Mann. They use swear words like "Damn" and "Hell" much more than the mercenaries. They only appear on one seasonal map, Helltower.

With this game having open chat/mic there will always be people that will say anything from crap to racial slurs. So there is no way to really gauge how bad it is.

Lone bottles and six-packs of beer can be lying around levels, but these are mere decorations and serve no other use.
A few of the Engineers taunts have beer.
A character is an alcoholic.
A character called demoman is an alcoholic, he can use a triple x alcohol bottle(most likely whisky) as a melee weapon and if he taunts while holding it he will take a sip, he also has many voice lines that seem to be drunkenly slurred
There is an unlockable primary for the Medic called "The Overdose" that makes you faster. This is a reference to drug overdosing.
The Spy is frequently shown smoking a cigarette, by default. The Soldier has an unlockable, optional cigar as an accessory players can add to his player model, and the Medic and Sniper likewise have an optional smoking pipe.
Two of the Demoman's melee weapons are bottles, one marked with "XXX", and the other reading "DeGroot Rum". Additionally, the Demoman has a number of drunk-sounding (slurred speech) voice responses triggered by events which occur occasionally. When the player taunts with any non-sword melee weapon, the Demoman says "Cheers, mate!" and drinks out of what he is holding, even if it isn't a bottle.

Pyro, one of the main characters may seem terrifying and cruel in a video clip, may scare younger audiences.
If this were a movie, it would've been rated R for violence and bloody images.
The game is a fast based movement shooter. This often leads to intense situations that can upset younger players.
There is a boss called "Saxton Hale" on certain maps. He can appear frightening due to his size (~1.5x the size of a normal player), his speed and how the screen shakes when he is close.
Parents may want to evaluate whether they want their children playing the game online, as this opens them up to scorn, language, and sexual references made by other players. Most servers, though, will ban players for doing these things, as well as the players can vote whether to kick the player or not. The actual game's content itself is fairly tame and comical.
The Halloween 2013 level/map Helltower contains an area underneath the map called "Hell", which is a large underground cavern with lava, ghosts, and a firey skull goal, which is accessed after the game completes on the surface. While on the surface, non-playable skeleton characters sometimes arise from the ground and attack players on both teams, or only players of one team, if summoned by an individual. This level involves both RED and BLUE teams racing to be the first to send the CEO of their opposing team to hell first.
Other Halloween bosses that could potentially be frightening are the Horseless Headless Horsemann, a skeletal figure in a cape who comically places on a pumpkin for a head, Monoculus, a giant flying eyeball that shoots rockets, and Merasmus, a large ghostly (but funny) wizard who casts spells and attacks both teams with bombs and his staff.
