The blood stains on the black Ford Thunderbird's headlights change repeatedly between shots during the chase.
At the beginning of the NO BRAKES scene at the top of the hill the main character's mustang has come to a stop and is shown leaking brake fluid. There is a shot of him looking down perplexed, only he is pushing the gas pedal instead of the brake. Apparently this shot was mistakenly inserted to Be a sign that the brakes were going bad. In context it seems to do the trick. However,closer examination shows it is the wrong pedal and should have resulted in the car accelerating, not sitting still, as is shown in the very next shot. Later, when the brakes are gone, he is using using the correct pedal.
When Trina falls onto the bed during her tryst near the end, she's still wearing the stocking on her head she'd taken off a moment earlier.
When Trina/Jade puts on her high heel in the hotel room, she puts on a standard high heel. For the rest of the scene, starting immediately with her "torture" of her client, she's shown wearing the high heels with ankle straps from a previous scene.

The footage that is recovered from the partially-burned Hi8 tape in the fire shows dust, dirt, and speckles upon playback. This kind of damage would only occur with a reel of film; a videotape would never exhibit film damage.
Just after the black Thunderbird runs over Patrice for the second time, somebody yells "Hey, you asshole!" Just over a minute later, as the Thunderbird skids through an intersection, the same voice can be heard yelling exactly the same line in the same way, indicating a repeated audio clip.

As Corelli leaves the crime scene at the beginning of the film, the black-and-white police car's large "SFPD" door emblem is an obvious cardboard fake; it has come unglued from the door and is curling up at the bottom.
When David is chasing the black car you can clearly see his airbag coming out with the second jump. In the next scene the airbag is gone.
In the opening scenes as the camera goes through the pictures on the desk and focuses on a picture of an individual with Ronald Reagan, a hand holding the camera over the glass picture frame is visible.
(at around 1h 18 min) Corelli uses the telephone at the murder site without using gloves or checking if the phone has been dusted for prints. Completely unacceptable for a seasoned prosecutor.
