Overview

Guidelines and format to follow when drafting, revising, or reviewing UCSF policies.

Policy Number and Title

The working title of the policy. Avoid using "UCSF" or "Policy" in the title. 

Policy Overview

Briefly summarize the content of the Policy. Ideally should be under 140 characters. May be similar to Purpose (see below).

Purpose

Succinctly summarize the intended purpose of the Policy. Ideally should be 3 or fewer sentences. Should answer the question: Why do we have this policy?

Definitions

List of specialized terms within the policy. Wherever possible, terms should be defined consistently across policies. 

Avoid defining a common term unless it has a specific and non-conventional use within the policy. Specialized terms which only appear once in the policy / procedure sections may be left out of the definitions section and defined where the term appears if space permits.

Policy Text

[This is the core purpose of the policy that states the governing principles that guides actions. It is intended to answer the question: “What are the rules / principles on this subject.” Policy should always be high level. Procedures are not to be included within policy.]

Responsibilities

List the individual roles by position or units/departments/committees (or Stakeholders) with required responsibilities or authorities for instituting or complying with all or some portion of the policy. Do not use names of individuals.

  • Which position is responsible for implementing and enforcing the policy?
  • Which position is responsible for providing interpretations/clarification?

Procedures

Procedures should be maintained separately from the policy. If needed, this section will comprise links to information such as forms and processes that are necessary for complying with this policy.

The Responsible Office must maintain any procedural pages. This enables updates to procedures without changing an entire policy.

[Reference any other sources, policies upon which this Policy is based on, such as federal and state laws (e.g., California Governor’s Executive Orders), or organizational documents that complement, supplement, or help explain provisions within the policy (e.g., guidelines, checklists, standards).
