The fans in the meat preparation room are very dirty; exposed meat is being packaged under these fans. Eggs that are for sale are stored in the aisle instead of in a refrigerated display case. Person in charge was told that the eggs need to be stored under refrigeration. Hose for sanitizer solution at the hot food 3-compartment sink is missing. Observed one deeply scored cutting boards not resurfaced or discarded as required. Discarded. Observed clean food equipment in kitchen area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting). Paper towel dispenser not working at the handwash sink in the deli and bakery service area.

Observed employee go from handling raw chicken to grabbing a container of seasoning without removing gloves and contaminating outside of container. Discussed handwashing procedures with operator and employee. Packaged foods in the Reach-in freezer in the kitchen area has accumulation of ice and in need of defrosting. Corn dip and Cooked Noodles, a refrigerated, ready to eat time temperature control for safety food in the reach-in fridge in the kitchen area, was date-marked by the facility, but was beyond the 7 day use or sell by date and requires discarding. Corn dip 12/4 and cooked noodles 12/11. Discarded. The quaternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 0 ppm, rather than 200 ppm as stated on the manufacturers use directions. Facility will use high temp dishwasher and tablets until three-compartment sink auto sanitizer is repaired. Plastic and metal pans, a food contact surface, was observed to have food residue and old sticker residue and was not clean to sight and touch. Corrected moved to dish.

Observed take-out containers in the service area and clean food equipment in the prep area, stored uncovered or not inverted. Take-out containers were correctly stored during the inspection. The handwash sink located in the front service area does not have water at a temperature of at least 100 degrees F. No sign or poster posted at the handwash sink in the front service area to remind food employees to wash their hands. The paper towel dispenser and soap dispenser are missing. Chemicals and OTC medicine were stored on a shelf with food items and equipment.
Latin House LLC, 433 N. 9th St. Lebanon, December 19. Pass. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration. The bottom of one container of food utensils, a food contact surface, was observed to have food residue and was not clean to sight and touch. Several non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil; specific items were the bottom shelf of a prep table, a wall and door frame.

Dragon Garden Restaurant, 535 Cumberland St. Lebanon, Complaint, December 18. Pass. No violations.
The Filling Station, 320 W. Main St. Palmyra, December 18. Pass. Two containers of strawberries and one lemon observed in the walk-in cooler area, spoiled/moldy and is adulterated. Discarded. Beef BBQ and sausage gravy was held at 90-119 degrees F, in the hot holding cabinet, rather than 135 degrees F or above as required. Products where removed and reheated, cabinet was turned up, ambient temp was holding at 121 degrees F. Interior sides of microwave on the cook-line next to toaster with food debris and in need of cleaning. Handwashing sink in the employee restroom is slow to drain/ clogged and needs to be repaired.
